THE SHORT ANSWER

Does Humana Cover Wegovy?

Weight Loss Is Excluded From Humana's Own Drug Benefit

Humana's own Medicare drug plans don't cover Wegovy for weight loss. Humana excludes weight loss as a covered indication for GLP-1 medications (a class of injectable weight loss drugs) in its plan benefit—this isn't a prior authorization denial, it's a plan exclusion that blocks claims at the pharmacy level.[1] But Humana Medicare members can still get Wegovy for weight loss through the Medicare GLP-1 Bridge, a separate CMS program: $50 per monthly fill, with conditions. (Humana no longer sells employer-group commercial medical plans (exit completed in 2025).)

Unlike some insurers where coverage "varies by plan," Humana has been unusually clear: GLP-1 agonists will not be covered for weight loss through the plan benefit of its MAPD/PDP plans (Medicare Advantage with drug coverage and standalone prescription drug plans). Here's what this means for you:

  • Your claim won't reach prior authorization—it will reject automatically at the pharmacy
  • Appealing is unlikely to help—you're fighting a plan exclusion, not a medical necessity denial
  • Other routes exist: the Medicare GLP-1 Bridge for weight loss ($50 per monthly fill, July 1, 2026 – December 31, 2027), Type 2 diabetes (via Ozempic), or some Medicaid plans

Bottom line: If you have Humana Medicare and want Wegovy for weight loss, ask your prescriber about the Medicare GLP-1 Bridge; otherwise you'll likely need self-pay options or other coverage pathways—not appeals.

WHAT IT ACTUALLY MEANS

Humana's Weight Loss Exclusion

Since June 2023, Humana has implemented a diagnosis code requirement for all GLP-1 medications. This is the mechanism that blocks weight loss coverage:

How the Diagnosis Code Edit Works

When your pharmacy submits a Wegovy claim, Humana's system checks the ICD-10 diagnosis code (the medical codes doctors use to describe your condition). If it's obesity or weight loss (E66.x codes), the claim automatically rejects—before any human reviews it, before any prior authorization request.

This isn't like a PA denial you can appeal. It's a hard stop in the system.

Accepted vs. Rejected Diagnosis Codes

Humana GLP-1 diagnosis code requirements
ICD-10 CodeDiagnosisClaim Result
E11.xxType 2 Diabetes MellitusMay Process (with PA)
E08.xx, E09.xx, E13.xxOther Diabetes TypesMay Process (with PA)
G47.33Obstructive Sleep ApneaLimited (Medicare only)
E66.01, E66.09, E66.1Obesity/OverweightAuto-Reject
Z68.xBMI CodesAuto-Reject

Humana's Rejection Messages

When a GLP-1 claim is rejected, you'll see one of these error codes:

  • Reject Code 39: "ICD-10 is required"—no diagnosis code was submitted
  • Reject Code 88: "Use is not medically accepted"—the obesity/weight loss code triggered the exclusion

WHERE WEGOVY MIGHT BE COVERED

Humana Plan Types

Coverage varies significantly by the type of Humana plan you have:

Humana plan types and Wegovy coverage
Plan TypeWeight LossDiabetesKey Notes
Commercial (Employer)No longer offered—Humana no longer sells employer-group commercial medical plans (exit completed in 2025)
Medicare Advantage (MAPD)Bridge: $50 per monthly fill (with conditions)May cover Ozempic for T2DBasic Part D coverage excludes weight-loss drugs (federal law); the Medicare GLP-1 Bridge is separate (outside Part D)
Medicare Part D (PDP)Bridge: $50 per monthly fill (with conditions)VariesSame federal exclusion applies to the plan; the Bridge is separate
Medicaid (varies by state)Some statesVariesCheck your state; states covering GLP-1s for obesity: 13 states (as of January 2026)

Sleep Apnea: Wegovy Has No Sleep Apnea Indication

Important: Wegovy has no FDA approval for sleep apnea—only Zepbound (tirzepatide) received that approval, on December 20, 2024.[2] So an OSA coverage pathway doesn’t apply to Wegovy on Humana Medicare.

If You Have Sleep Apnea

  • Zepbound, not Wegovy: Your Part D plan may cover Zepbound for moderate-to-severe sleep apnea, usually with prior authorization. Ask your doctor whether it fits you.
  • Bridge exclusion: the Medicare GLP-1 Bridge does not cover people with moderate-to-severe sleep apnea (Not eligible if you have type 2 diabetes, moderate-to-severe obstructive sleep apnea, or noncirrhotic MASH with moderate-to-advanced liver fibrosis (F2–F3), or if your Part D plan paid for a GLP-1 for you at any time in 2026).

How to Check Your Specific Plan

Call Script for Humana

"Hi, I'm calling to check coverage for a prescription medication. My member ID is [NUMBER]. I need to know:

  1. Is Wegovy (semaglutide) covered on my plan's formulary?
  2. Is it covered for weight management, or only for diabetes or heart disease?
  3. What diagnosis codes are accepted for this medication?
  4. If it's not covered for weight loss, what are my options?"

UNDERSTAND YOUR OPTIONS

If Humana Denies Wegovy

Important: Plan Exclusions Rarely Overturn

If Humana denied your Wegovy because weight loss isn't a covered indication, an appeal is unlikely to succeed. You're fighting the plan's benefit structure, not a medical necessity decision. Save your energy for cases where appeals make sense.

Common Humana Denial Reasons

  • "Plan exclusion" — Weight loss is not a covered indication (most common)
  • "Missing diagnosis code" — Pharmacy didn't submit ICD-10 code (resubmit, don't appeal)
  • "Prior auth not obtained" — Medicaid plans require PA before filling
  • "Step therapy required" — Must try other meds first (Saxenda, Contrave)

When an Appeal Makes Sense

  • Medicaid PA denial — you meet criteria but were denied anyway
  • CV-risk coverage dispute — Part D denied Wegovy prescribed for cardiovascular risk reduction despite established cardiovascular disease
  • Wrong diagnosis code — your doctor listed the wrong indication
  • Renewal denial — you showed ≥5% weight loss but were still denied

If your situation fits one of these, appeals can work. See Humana's official appeals process[6] or use Honest Care to build your appeal →

Humana Medicaid (Exception)

If you have Humana Healthy Horizons (Medicaid) in a state that covers GLP-1s for weight loss, prior authorization applies. States covering GLP-1s for obesity under Medicaid: 13 states (as of January 2026).[3]

For complete prior auth preparation, see our Wegovy prior authorization guide → and Novo Nordisk's official PA resource guide.[5]

What If Humana Won't Cover Wegovy?

Since Humana explicitly excludes weight loss coverage on most plans, here are realistic alternatives:

Alternative options for Humana members
OptionCostNotes
NovoCare (injectable)$199–$349/month$199/month for the first two fills (0.25 mg and 0.5 mg), through December 31, 2026; standard doses $349/month ($399 for Wegovy HD 7.2 mg)[7]
Oral Wegovy pill (new Jan 2026)$149–$299/month by dose$149 (1.5 mg), $199 (4 mg), $299 (9 mg and 25 mg) via NovoCare[8]
Retail pharmacy (no coverage)~$1,600/monthAverage retail ~$1,615; GoodRx coupons may reduce cost[9]
Alternative GLP-1sVariesCheck if Zepbound, Saxenda, or Ozempic have different coverage

What About the Wegovy Savings Card?

The savings card (as little as $25) requires commercial insurance that covers Wegovy—so it won't work with Humana's Medicare plans. No current Wegovy card price for plans that don’t cover it; self-pay pricing applies: $349/month ($399 for Wegovy HD 7.2 mg) through NovoCare. If you have government insurance, ask NovoCare whether you can use self-pay pricing.

COMMON QUESTIONS

Humana Wegovy FAQs

Not through Humana's own plan benefit—but Humana Medicare members can use the Medicare GLP-1 Bridge. Humana excludes weight loss as a covered indication for GLP-1 medications on its Medicare drug plans. Claims with obesity diagnosis codes are automatically rejected at the pharmacy level; that's a plan exclusion, so appeals are unlikely to succeed. Separately, the CMS-run Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027) covers Wegovy for weight loss for $50 per monthly fill if you meet CMS's criteria and your prescriber gets prior authorization. Humana no longer sells employer-group commercial medical plans (exit completed in 2025).

Not Wegovy specifically—Wegovy is FDA-approved for weight management and cardiovascular risk reduction, not diabetes. However, Ozempic (same active ingredient, semaglutide) is approved for Type 2 diabetes and may be covered on Humana plans with prior authorization. Ask your doctor about Ozempic if you have Type 2 diabetes.

No. Wegovy has no FDA approval for sleep apnea, so a sleep apnea diagnosis doesn't create a Wegovy coverage pathway. Zepbound is the GLP-1 approved for sleep apnea: Your Part D plan may cover Zepbound for moderate-to-severe sleep apnea, usually with prior authorization. Note that people with moderate-to-severe sleep apnea can't use the Medicare GLP-1 Bridge.

Most likely because the prescription included an obesity/weight loss diagnosis code. Since June 2023, Humana requires pharmacies to submit ICD-10 codes for GLP-1 claims. If the code indicates obesity (E66.x) rather than diabetes (E11.x), the claim auto-rejects with "Reject Code 88: Use is not medically accepted."

You can, but it's unlikely to succeed for plan exclusions. If Humana denied coverage because weight loss isn't a covered indication, you're fighting the plan's benefit structure—not a medical necessity decision. Appeals are more effective for: Medicaid PA denials where you meet criteria, CV-risk coverage disputes, or renewal denials where you demonstrated required weight loss.

It varies by state. Humana operates Medicaid plans (Humana Healthy Horizons) in FL, KY, LA, IN, WI, IL, OH, and SC. State Medicaid programs covering GLP-1s for obesity: 13 states (as of January 2026). Contact your state's Humana Medicaid plan directly to verify current coverage and PA requirements.

If you have Humana Medicare, start with the Medicare GLP-1 Bridge ($50 per monthly fill, with conditions). Other options: NovoCare self-pay ($349/month ($399 for Wegovy HD 7.2 mg) for the injectable, $149–$299/month by dose for the new oral pill; if you have government insurance, ask NovoCare whether you can use self-pay pricing), or checking if alternative GLP-1s have different coverage on your plan. The savings card requires commercial insurance that covers Wegovy.

WHAT'S COMING

Humana Coverage Outlook for 2026

Humana's weight loss exclusion is expected to continue through 2026. However, several developments may affect your options:

  • Medicare GLP-1 Bridge is live (July 1, 2026 – December 31, 2027) — $50 per monthly fill for Humana MAPD and PDP members who qualify. Part D plans and pharmacies don’t opt in, and you don’t enroll separately; your prescriber must get prior authorization.
  • Oral Wegovy launched January 2026 — Available via NovoCare at $149–$299/month by dose, no injection required
  • Medicaid expansion — Some states adding GLP-1 coverage; check your state's Humana Healthy Horizons plan
