PRIOR AUTHORIZATION BASICS

Quick Answer

Short answer: Most employer and ACA plans must decide a Wegovy prior authorization within 15 days (72 hours if urgent), and many decide sooner. Most insurers require BMI ≥30 (or ≥27 with a qualifying condition), documented lifestyle attempts, and sometimes prior medication trials—but requirements vary by plan.

What do I need for Wegovy prior authorization?

  • BMI documentation: ≥30 or ≥27 with a weight-related health condition (high blood pressure, diabetes, sleep apnea, heart disease)
  • Lifestyle modification: 3-6 months of documented diet/exercise
  • Step therapy: Some insurers require trying cheaper meds first

Who this guide is for

Patients starting the PA process, waiting on a decision, or recently denied and considering next steps.

UNDERSTANDING THE BASICS

What Is Prior Authorization?

Prior authorization (PA) is insurance approval required before your pharmacy can fill your Wegovy prescription. Your insurer reviews whether you meet their medical necessity criteria—their standards for deciding if a treatment is medically needed for you.[1]

They check things like your BMI (body mass index), comorbidities (other health conditions like diabetes or high blood pressure), and sometimes prior medication trials. This is standard for GLP-1 (glucagon-like peptide-1) weight loss medications—it's not a denial.

Key Point

You can't submit the PA yourself—your doctor's office handles it. But you can prepare documentation ahead of time and follow up to speed the process.

Think of it this way: your doctor writes the prescription, but your insurance company needs to verify you qualify under their guidelines before they'll pay for it. For most employer and ACA plans, federal rules require a decision within 15 days (72 hours if urgent); complete documentation helps avoid delays.[4]

Why Do Insurers Require PA for Wegovy?

  • Cost control: Wegovy costs $1,349+/month without insurance.[3] Insurers want to verify medical necessity.
  • Verify eligibility: They confirm you meet FDA-approved criteria (BMI ≥30 or ≥27 with comorbidity).[1]
  • Step therapy: Some insurers require you to try cheaper medications first.

STEP BY STEP

The PA Process

Here's what happens after your doctor writes a Wegovy prescription:

1

Doctor Writes Prescription

Your provider sends the Rx to your pharmacy. This starts the process.

2

Pharmacy Attempts to Fill

The pharmacy runs it through your insurance. The system automatically flags that PA is required.

3

Doctor's Office Submits PA

Your provider's office is notified and submits the PA request with your medical documentation. You don't submit this directly.

4

Insurer Reviews

The insurance company checks if you meet their specific criteria for Wegovy coverage.

5

Decision Communicated

You'll get approved, denied, or they'll request more information. Your doctor's office usually hears first.

Timeline Expectations

Prior authorization timeline expectations
ScenarioExpected TimelineYour Action
Complete documentationOften faster than the legal maximum; varies by planWait for notification
Standard (non-urgent) requestUp to 15 days (one 15-day extension allowed) under federal rules for most employer and ACA plans[4]Follow up after 5 days
Missing info/complexThe plan must say what's missing; you get at least 45 days to send itProvide requested docs ASAP
Urgent request72 hours (federal rule for urgent requests)[4]Ask doctor to flag as urgent

Speed Tip

Incomplete documentation is the #1 cause of delays. Come to your appointment prepared with your weight history, comorbidity diagnoses, and prior weight loss attempts documented.

PRIOR AUTH REQUIREMENTS

What You Need to Qualify

While exact criteria vary by insurer, here are the standard requirements for Wegovy prior authorization. Some insurers also require step therapy (trying other medications first). See detailed requirements by insurer →

Standard Wegovy prior authorization requirements
RequirementStandard ThresholdNotes
BMI≥30 (obese) OR ≥27 with comorbidityUHC may require ≥40 for some plans
Qualifying comorbiditiesHigh blood pressure, type 2 diabetes, high cholesterol, sleep apnea, cardiovascular diseaseAt least one if BMI 27-29.9
Lifestyle modification3-6 months documented (varies)BCBS may require 6 months in program
Step therapyVaries by insurerMay require prior med trials

Qualifying Comorbidities

These health conditions qualify you for Wegovy coverage at BMI 27-29.9. Make sure your medical records include the proper diagnosis codes (ICD-10 codes are the standard billing codes doctors use):[2]

  • High blood pressure — code I10
  • Type 2 diabetes — code E11
  • High cholesterol (dyslipidemia) — code E78.5
  • Sleep apnea (obstructive) — code G47.33
  • Cardiovascular disease — codes I20-I25

What Is Step Therapy?

Some insurers require you to try cheaper weight loss medications first before approving Wegovy. Common step therapy drugs include:

  • Phentermine — 3 months typical trial
  • Contrave (naltrexone/bupropion) — 3 months
  • Qsymia (phentermine/topiramate) — 3 months
  • Orlistat — 3 months

Already Tried These?

If you've previously tried weight loss medications, document: dates, doses, duration, outcomes, and why you stopped. Had side effects or contraindications? Note those too—this can satisfy step therapy requirements.

Step therapy is one of the most common reasons for delays. Come prepared with your prior medication history.

CVS Caremark Advantage

Good news: Wegovy is CVS Caremark's preferred GLP-1 for weight loss. When CVS Caremark made Zepbound nonpreferred or excluded on some (its most common) commercial template formularies (lists of covered drugs) in July 2025, it kept Wegovy as the preferred option. That can make approval easier; check your plan’s criteria for any step therapy.[5]

UHC Coverage Note

UnitedHealthcare's Wegovy criteria may be stricter than Zepbound—some UHC plans require BMI ≥40 for Wegovy compared to ≥30 for Zepbound. UHC also gives Wegovy a 5-month initial approval vs 6 months for Zepbound. Always verify your specific plan's coverage.[6]

Requirements change frequently. Always verify current criteria with your specific plan.

WEGOVY IS PREFERRED

CVS Caremark Advantage

July 2025 Update: Wegovy Wins

When CVS Caremark made Zepbound nonpreferred or excluded on some (its most common) commercial template formularies in July 2025, Wegovy became the preferred GLP-1 for weight loss. Since October 1, 2026, Zepbound is back on CVS Caremark’s commercial template formularies as an additional preferred option (plan sponsors can customize). If you have CVS Caremark, Wegovy remains a preferred option with a standard PA process.[5]

If You're on CVS Caremark

Already on Wegovy?

You're in the best position—continue with your current PA and enjoy preferred status. Renewals should be straightforward.

Currently on Zepbound?

Since October 1, 2026, Zepbound is back on CVS Caremark’s commercial template formularies as an additional preferred option (plan sponsors can customize), so you may not need to switch. Check your plan’s formulary; if your plan still excludes Zepbound, your doctor can help you transition to Wegovy—same drug class (GLP-1), similar efficacy.

Starting fresh?

Request Wegovy first—it's the path of least resistance with CVS Caremark. Your PA is more likely to be approved without step therapy hurdles.

WEGOVY'S UNIQUE ADVANTAGE

Cardiovascular Pathway

FDA-Approved for Heart Health

On March 8, 2024 (Wegovy injection), the FDA approved Wegovy injection for cardiovascular risk reduction in adults with established cardiovascular disease and either obesity or overweight (the tablets got the same indication with their approval, December 22, 2025 (Wegovy tablets))—making it the first weight loss drug with a heart protection indication.[7]

The SELECT trial (17,604 participants) showed Wegovy reduced major cardiovascular events—heart attacks, strokes, and cardiovascular death—by 20% compared to placebo.[7] This opens important coverage pathways.

Why This Matters for Coverage

Wegovy coverage pathways
PathwayWho QualifiesCoverage Implications
Weight LossBMI ≥30 or ≥27 with comorbidityStandard PA; Medicare: basic Part D coverage excludes it, but the Medicare GLP-1 Bridge covers it ($50 per monthly fill, with conditions)
CV Risk ReductionEstablished heart disease + BMI ≥27May qualify for Medicare Part D
MASH (Liver Disease)Noncirrhotic MASH with F2-F3 fibrosisMay qualify for Medicare Part D

Medicare Part D Pathway

By law, basic Medicare Part D coverage excludes drugs used for weight loss; for weight loss, the Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027), which runs outside the Part D benefit, covers Wegovy for weight loss for $50 per monthly fill if you meet CMS’s criteria and your prescriber gets prior authorization. Wegovy's CV indication provides a potential Part D pathway:

  • If you have documented cardiovascular disease (prior heart attack, stroke, or peripheral artery disease) AND
  • BMI ≥27 (no diabetes required)

Your doctor may prescribe Wegovy for cardiovascular risk reduction rather than weight loss. This may qualify for Medicare Part D coverage under the CV indication.[8]

Important Caveat

Medicare coverage for Wegovy under the CV indication is not guaranteed—coverage depends on the plan’s formulary, prior authorization and exceptions process. Check with your specific Part D plan.

Commercial Plans: CV Pathway May Be Easier

Even on commercial insurance, the CV indication can help if your plan excludes weight loss drugs:

  • Different PA criteria: CV indication may have different (sometimes easier) requirements
  • Bypass weight loss exclusions: Some plans that exclude obesity drugs still cover cardiovascular medications
  • Stronger medical necessity case: Heart disease documentation strengthens your appeal

Had a heart attack, stroke or peripheral artery disease? Talk to your doctor about whether the CV indication pathway is right for you.

LIVER DISEASE INDICATION

MASH Pathway

FDA-Approved for Fatty Liver Disease (August 2025)

On August 15, 2025, the FDA approved Wegovy for treating MASH (metabolic dysfunction-associated steatohepatitis)—a serious form of fatty liver disease affecting ~15 million Americans. Wegovy is the first GLP-1 approved for MASH.[11]

The ESSENCE trial showed 63% of patients on Wegovy had MASH resolution (vs 34% on placebo), and 37% had improvement in liver fibrosis (vs 22% on placebo). This opens another important coverage pathway.

Who Qualifies for the MASH Pathway?

  • Diagnosed with MASH (not just fatty liver/NAFLD—must be steatohepatitis)
  • Moderate to advanced fibrosis (stages F2-F3, confirmed by FibroScan or biopsy)
  • No cirrhosis (the approval is for noncirrhotic MASH only)

Why This Matters for Coverage

  • Bypass weight loss exclusions: Plans that exclude obesity drugs may cover liver disease treatments
  • Medicare Part D potential: Like the CV indication, MASH may provide a pathway around the weight loss drug exclusion
  • Different PA criteria: Liver disease documentation (FibroScan, biopsy, liver enzymes) may be easier to obtain than weight loss program history

Do You Have MASH?

MASH is often undiagnosed. If you have obesity plus elevated liver enzymes (ALT, AST) or fatty liver on imaging, ask your doctor about testing for MASH. A FibroScan can assess fibrosis stage non-invasively. If you qualify, the MASH indication may provide an easier path to coverage.

DON'T GIVE UP

What If Denied?

Appeals Work

Over 65% of appealed commercial insurance denials are overturned when properly documented[9]—yet fewer than 1 in 10 patients ever appeal. Most plans give you at least 180 days to file an internal appeal (check your denial letter for your plan's exact deadline).[4]

A denial isn't the end—it's a starting point. Here's what to do:

  1. Get your denial letter with the specific reason
  2. Identify what was missing from your documentation
  3. Gather additional evidence (labs, records, letter of medical necessity)
  4. Submit internal appeal within your plan's deadline (typically 180 days for most plans)
  5. If denied again → Request external review (40-60% success rate)[4]

Common Denial Reasons & How to Appeal

Click your denial reason for specific appeal strategies:

Need Wegovy while your appeal is processing? Many patients use NovoCare self-pay ($199–$349/mo) to start treatment while waiting—then switch to insurance once approved.

Plan excludes Wegovy entirely? Other GLP-1s like Ozempic (same active ingredient, semaglutide) may be covered for type 2 diabetes. Compare coverage options →

Alternative: Cardiovascular Pathway

If you have documented cardiovascular disease and obesity or overweight, Wegovy is FDA-approved for CV risk reduction and may have different PA requirements. Some plans that exclude weight loss still cover CV medications. Learn about the CV pathway →

WHAT'S CHANGING

Prior Authorization Outlook for 2026

Mixed News for 2026

While some insurers are tightening GLP-1 coverage, Wegovy has unique advantages: CVS Caremark preferred status, CV indication for Medicare, and the new oral form. These factors may make Wegovy approval easier than competitors in 2026.

Confirmed for 2026

Oral Wegovy Available (December 2025)

The FDA approved oral Wegovy (semaglutide tablets) on December 22, 2025. The pill form costs as little as $25/month with insurance + savings card, or $149–$299/month self-pay via NovoCare. This provides a new option if you prefer tablets over injections.[10] (Pricing as of January 2026; check NovoCare.com for current offers.)

CVS Caremark Keeps Wegovy Preferred

Wegovy remains a preferred GLP-1 on CVS Caremark formularies. Since October 1, 2026, Zepbound is back on CVS Caremark’s commercial template formularies as an additional preferred option (plan sponsors can customize), so standard CVS Caremark formularies now list two preferred options; plans can customize, so check yours.

Medicare GLP-1 Bridge (Live)

The Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027) is live and covers Wegovy for weight loss for $50 per monthly fill if you meet CMS’s criteria and your prescriber gets prior authorization. What comes after it is not settled: No successor announced after December 31, 2027. Wegovy's CV indication is a separate Part D pathway.[8]

Some State Medicaid Dropping Coverage

California Medi-Cal will no longer cover GLP-1s for weight loss effective January 1, 2026, with limited exceptions for patients under age 21. Other states may follow.

Expected PA Criteria (2026)

Standard Requirements Continue

Most commercial insurers expected to maintain current requirements:

  • BMI ≥30 or ≥27 with comorbidity
  • 3-6 months documented lifestyle modification
  • ≥5% weight loss for reauthorization
  • 6-month initial authorization periods

What Should You Do Now?

  • Verify 2026 coverage: Don't assume your plan will cover Wegovy next year—check during open enrollment
  • Consider CVS Caremark: If switching plans, CVS Caremark lists Wegovy as preferred (and, since October 1, 2026, Zepbound too)
  • Explore CV pathway: If you have established cardiovascular disease, the CV indication may offer a different approval route
  • Try oral Wegovy: The new pill form may have different PA requirements and is more affordable
  • Budget for self-pay: NovoCare pricing ($199–$349/mo) is available now

COMMON PRIOR AUTH QUESTIONS

Frequently Asked Questions

Prior authorization (PA) is insurance approval required before your pharmacy can fill your prescription. The insurer verifies you meet their medical necessity criteria—BMI, comorbidities, and sometimes prior medication trials—before they'll cover the medication.

For most employer and ACA plans, federal rules require a decision within 15 days (one 15-day extension allowed), and many plans decide sooner when documentation is complete. If medically urgent, your doctor can request an expedited review, which must be decided within 72 hours.

Per FDA labeling: BMI ≥30 (obese) or BMI ≥27 with at least one weight-related comorbidity (hypertension, type 2 diabetes, dyslipidemia, OSA, cardiovascular disease). Note: Some UHC plans may require BMI ≥40.

Hypertension, type 2 diabetes, dyslipidemia (high cholesterol), obstructive sleep apnea (OSA), and cardiovascular disease (including prior heart attack or stroke). The CV comorbidity is especially important for Wegovy due to its FDA-approved cardiovascular indication.

Your doctor's office submits the PA—you cannot submit it directly. But you can prepare documentation ahead of time and follow up with your provider to speed the process.

Yes—Wegovy is a preferred GLP-1 for weight loss on CVS Caremark's commercial template formularies. From July 2025 to September 2026, Zepbound was nonpreferred or excluded on some of them; Since October 1, 2026, Zepbound is back on CVS Caremark’s commercial template formularies as an additional preferred option (plan sponsors can customize). Preferred status can make PA approval easier; check your plan's criteria for any step therapy.

Yes, with conditions, through the Medicare GLP-1 Bridge, a temporary CMS program that runs outside your Part D benefit. By law, basic Part D coverage excludes drugs used for weight loss, but the Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027) covers Wegovy for weight loss for $50 per monthly fill if you meet CMS's criteria and your prescriber gets prior authorization. Wegovy is also FDA-approved for cardiovascular risk reduction in adults with established cardiovascular disease and either obesity or overweight; for that use it may be covered under Part D, subject to the plan's formulary, prior authorization and exceptions process.

On March 8, 2024 (Wegovy injection), the FDA approved Wegovy injection to reduce the risk of major cardiovascular events (heart attack, stroke, CV death) in adults with established cardiovascular disease who have overweight or obesity (the tablets got the same indication on December 22, 2025 (Wegovy tablets)). The SELECT trial showed a 20% reduction in these events. This is separate from the weight loss indication and may open coverage pathways on plans that exclude obesity drugs.

Yes—the FDA approved oral Wegovy (semaglutide tablets) on December 22, 2025. The pill costs as little as $25/month with insurance + savings card, or $149–$299/month self-pay. This provides an alternative if you prefer not to inject.

Most insurers require at least 5% weight loss from baseline for reauthorization. If you plateau, document all circumstances and appeal—success isn't guaranteed but is possible with strong documentation of ongoing lifestyle modification and other benefits (improved blood pressure, A1C, etc.).

Yes—and you should. Over 65% of appealed commercial insurance denials are overturned when properly documented—yet fewer than 1 in 10 patients ever appeal. You typically have 180 days to file an internal appeal (check your denial letter for exact deadline).

Wegovy and Ozempic contain the same active ingredient (semaglutide) but are FDA-approved for different uses. Ozempic is for type 2 diabetes; Wegovy is for weight loss and CV risk reduction. If your plan covers Ozempic for diabetes but not Wegovy, discuss with your doctor whether Ozempic might work for you.
