THE SHORT ANSWER

Does UnitedHealthcare Cover Wegovy?

Coverage Is Not Guaranteed

Whether a UnitedHealthcare commercial plan covers Wegovy for weight loss: Depends on your employer’s plan. Coverage depends on whether your employer added weight management benefits. Even when covered, strict prior authorization is required.[1]

UnitedHealthcare (UHC) is the largest health insurer in the United States, but that doesn't mean Wegovy coverage is standard. Here's what you need to know:

  • Employer-sponsored plans only cover Wegovy if the employer specifically added weight management drug benefits[1]
  • Individual/Marketplace plans typically exclude weight loss medications entirely
  • Medicare Advantage plans' own drug benefit excludes Wegovy for weight loss (basic Part D coverage excludes weight-loss drugs), but members can use the Medicare GLP-1 Bridge ($50 per monthly fill, with conditions) — see our Medicare coverage guide[2]
  • Optum Rx (UHC's pharmacy benefit manager) places Wegovy in high-cost specialty tiers when covered

Bottom line: Don't assume your UHC plan covers Wegovy. Call the member services number on your insurance card and ask specifically about Wegovy (semaglutide) coverage for weight management.

For a complete overview of getting Wegovy covered regardless of insurer, see our full coverage guide.

UHC'S PHARMACY BENEFIT MANAGER

Does OptumRx Cover Wegovy?

OptumRx = UHC's PBM

OptumRx is UnitedHealthcare's pharmacy benefit manager (PBM)—the company that manages prescription drug coverage for UHC plans. If you have UnitedHealthcare insurance, OptumRx handles your medication formulary (approved drug list) and prior authorizations.

Does OptumRx cover Wegovy? The answer depends on your specific UHC plan:

  • Employer plans with GLP-1 (a class of weight loss medications) coverage: Yes, OptumRx will process Wegovy with prior authorization (pre-approval from your insurer)
  • Employer plans without GLP-1 coverage: No, OptumRx will show "not covered" or "excluded" at the pharmacy
  • Individual/Marketplace plans: Rarely covered through OptumRx
  • Medicare Part D (via OptumRx): The plan's basic Part D coverage excludes weight loss, but the Medicare GLP-1 Bridge (outside Part D) covers Wegovy for $50 per monthly fill (with conditions) — see Medicare guide

How to Check Your OptumRx Coverage

  1. Log into OptumRx at optumrx.com or the OptumRx app
  2. Search for "Wegovy" in the drug lookup/formulary tool
  3. Check coverage status: It will show "covered with prior auth," "not covered," or "excluded"
  4. Call Member Services: For detailed questions, call the number on your UHC ID card

Key point: "OptumRx coverage" and "UHC coverage" are the same thing for prescription drugs. OptumRx is simply the entity that processes your UnitedHealthcare pharmacy benefits. All the UHC coverage rules, prior authorization requirements, and appeal processes described on this page apply to OptumRx.

HOW COVERAGE VARIES

UHC Plan Types & Optum Rx Formulary

Your specific UHC plan type significantly affects whether Wegovy might be covered:

UHC plan types and typical Wegovy coverage
Plan TypeCoverage LikelihoodNotes
Employer Large GroupVaries widelySelf-funded employers can choose to cover or exclude; ask HR
Employer Small GroupUsually excludedFully-insured plans rarely cover weight loss drugs
Individual/ACA MarketplaceRarely coveredMost individual plans exclude weight loss medications
UHC Medicare AdvantageBridge: $50 per monthly fill (with conditions)The plan's own Part D benefit excludes weight loss; the Medicare GLP-1 Bridge is separate. See Medicare guide[2]
UHC MedicaidState-dependentCoverage varies by state; most exclude weight loss drugs

Optum Rx Formulary Placement

Optum Rx is UnitedHealthcare's pharmacy benefit manager (the company that handles prescription coverage). For commercial plans that do cover Wegovy:

  • Where it falls on the drug list: Typically not on the standard drug list, or placed in the highest-cost specialty tier
  • Prior authorization: Always required—your doctor must get approval before you can fill it
  • Cost: Even when covered, expect high out-of-pocket costs due to specialty tier placement

Note: Coverage varies by specific plan. Always check your plan's drug list at uhc.com or by calling member services.

How to Check Your Specific Coverage

Call Script for UHC/Optum

"Hi, I'm calling to check coverage for a prescription medication. My member ID is [NUMBER]. I need to know:

  1. Is Wegovy (semaglutide) covered on my plan for weight management?
  2. What tier is it on? What would my copay be?
  3. Is prior authorization required?
  4. What are the coverage criteria—BMI (body mass index), required health conditions, anything else?
  5. Can you send me the coverage policy in writing?"

Pro tip: Always request coverage criteria in writing. Verbal confirmations aren't binding, and having documentation helps if you need to appeal later.

UHC CRITERIA FOR WEGOVY

Prior Authorization Requirements

When UHC does cover Wegovy, prior authorization is always required. Key criteria from UHC's official policy (P 1114-18):[3]

  • BMI ≥30, or ≥27 with a comorbidity (a weight-related health condition like diabetes, high blood pressure, sleep apnea, or high cholesterol)
  • Age 12+ (adults or adolescents with BMI ≥95th percentile)
  • Lifestyle modifications documented (diet, exercise, behavioral support)
  • Initial approval: 5 months; Renewals: 12 months (must show ≥5% weight loss)

UHC Does NOT Require Step Therapy

Unlike some insurers, UHC does not require you to try and fail other weight loss medications before Wegovy is approved. You must still meet BMI criteria and show lifestyle modifications.[3]

For complete prior auth preparation and checklists, see our Wegovy prior authorization guide →

UNDERSTANDING YOUR OPTIONS

Sleep Apnea & GLP-1 Coverage

Important Clarification: Wegovy Is NOT FDA-Approved for Sleep Apnea

On December 20, 2024, the FDA approved Zepbound (tirzepatide)—not Wegovy—as the first medication for moderate-to-severe obstructive sleep apnea in adults with obesity.[8] If you have OSA, ask your doctor if Zepbound may be appropriate.

UHC's Sleep Apnea Policy Covers Zepbound, Not Wegovy

UHC’s commercial sleep apnea policy (P 1475-3) is Zepbound-only: BMI ≥30 and an AHI, REI or RDI over 15, plus other conditions; 6-month first approval. Wegovy has no FDA-approved sleep apnea indication, so this OSA pathway doesn’t apply to Wegovy.[4]

Bottom line: If you have OSA and want a GLP-1, ask your doctor whether Zepbound, the GLP-1 FDA-approved for this indication, is appropriate. For Wegovy, coverage depends on your plan's weight-management benefit (or Wegovy's cardiovascular indication).

DON'T GIVE UP

If UHC Denies Wegovy

Good News: Most Appeals Succeed

65%+ of commercial insurance denials get overturned when patients appeal. Yet fewer than 1 in 10 patients ever file an appeal—those who do have a real chance of getting coverage.[5]

If your UHC plan denies Wegovy coverage, here's what to do:

Common UHC Denial Reasons

  • "Not medically necessary" — Documentation doesn't support need
  • "Does not meet criteria" — BMI or comorbidity requirements not documented
  • "Lifestyle modification not documented" — Need diet/exercise records
  • "Excluded benefit" — Plan categorically excludes weight loss drugs

What to Do:

1

Get the Denial in Writing

Request a written denial letter with the specific reason for denial. Note the exact denial code—you'll need this for your appeal.

2

Identify What's Missing

Compare your documentation to UHC's criteria. Common gaps: BMI not recent, lifestyle attempts not documented, conditions not properly coded.

3

Gather Additional Evidence

Work with your doctor to address the specific denial reason. Get updated documentation, lab results, or a Letter of Medical Necessity.

4

File Your Appeal

You have 180 days to appeal.[7] Need help? Use Honest Care to build your appeal →

Alternative Pathways

"Excluded benefit" denial? If your UHC plan categorically excludes weight loss medications, an appeal for medical necessity likely won't succeed. Consider these alternatives:

Have an Employer Plan? Talk to HR

Self-funded employers can modify coverage mid-year. Ask HR to advocate for adding GLP-1 coverage at the next plan renewal.

Open Enrollment

If your plan excludes Wegovy and appeals won't work, open enrollment is your opportunity to switch plans. Verify Wegovy coverage in writing before switching.

Just got denied? See our Wegovy appeals guide → for step-by-step strategies to overturn your denial.

WHAT'S CHANGING

UHC Coverage Outlook for 2026

Good news: UHC and OptumRx are maintaining Wegovy on 2026 formularies for plans that currently cover obesity medications. For Medicare Advantage members, see our Medicare coverage guide for 2026 policy updates.

Key 2026 Developments for Wegovy

NEW: Oral Wegovy Available January 2026

The FDA approved oral Wegovy (semaglutide pill) on December 22, 2025. Starting January 2026, you may have an alternative to the injection:

  • Daily pill instead of weekly injection
  • Up to 16.6% weight loss in OASIS 4 trial (13.6% average[10]; vs 14.9% for injection[6])
  • $149/month starting dose through NovoCare Pharmacy
  • Coverage policies still developing—check with UHC for formulary updates

Learn more about oral Wegovy →

CV Risk Reduction: Alternative Coverage Pathway

Wegovy is the first weight-loss medication approved to reduce cardiovascular risk (FDA, March 8, 2024, Wegovy injection), in adults with established cardiovascular disease and either obesity or overweight (injection: March 8, 2024 (Wegovy injection); tablets: December 22, 2025 (Wegovy tablets)).[9] If your UHC plan excludes weight loss drugs but you have established cardiovascular disease, ask your doctor about prescribing Wegovy for CV risk reduction instead—this may open a different coverage pathway.

What to Watch

  • Oral Wegovy formulary updates — UHC is evaluating the new pill form
  • Medicare — The Medicare GLP-1 Bridge is live (July 1, 2026 – December 31, 2027); the Part D BALANCE Model: Delayed, with no new start date
  • Employer plan renewals — annual renewals may add or drop GLP-1 coverage

UHC WEGOVY COVERAGE

Frequently Asked Questions

OptumRx is UnitedHealthcare's pharmacy benefit manager (PBM), so "OptumRx coverage" depends entirely on your UHC plan. If your UHC employer plan covers GLP-1 medications, OptumRx will process Wegovy with prior authorization. If your plan excludes weight loss drugs, OptumRx will show "not covered" at the pharmacy. OptumRx Medicare plans do not cover Wegovy for weight loss — see our Medicare guide for workarounds. Check your specific coverage at optumrx.com or call the number on your UHC ID card.

It depends on your specific plan. UHC's weight loss coverage is an optional program that employers must elect, so commercial coverage: depends on your employer’s plan. UHC Medicare Advantage plans' own drug benefit excludes Wegovy for weight loss (basic Part D coverage excludes weight-loss drugs), but members can use the Medicare GLP-1 Bridge ($50 per monthly fill, with conditions) — see our Medicare guide for details. Always call UHC member services to verify your specific coverage.

Per UHC's official policy, requirements include: BMI of 30+, or BMI of 27+ with a weight-related health condition (high blood pressure, type 2 diabetes, high cholesterol, or sleep apnea), age 12 or older (adolescents need BMI ≥95th percentile), and evidence of lifestyle changes (diet, exercise, behavioral support). Initial approval is for 5 months; to continue, you'll need to show at least 5% weight loss.

No. Unlike some insurers, UHC's official policy does not require you to try and fail other weight loss medications before Wegovy is approved. You must still meet BMI criteria and show you've tried lifestyle changes (diet, exercise, behavioral support).

You have 6 months (180 days) to file an internal appeal. Request the denial in writing with the specific reason, then submit an appeal addressing that reason with supporting documentation. 65%+ of commercial insurance denials are overturned when patients appeal—but fewer than 1 in 10 ever do. If the internal appeal fails, you can request an external review within 4 months—the decision is legally binding on UHC.

No. Wegovy is not FDA-approved for sleep apnea—that approval went to Zepbound (tirzepatide) on December 20, 2024. UHC’s commercial sleep apnea policy (P 1475-3) is Zepbound-only: BMI ≥30 and an AHI, REI or RDI over 15, plus other conditions; 6-month first approval. Wegovy has no FDA-approved sleep apnea indication, so this OSA pathway doesn’t apply to Wegovy. If you have OSA, ask your doctor if FDA-approved Zepbound might be a better fit.

For most employer and ACA plans, federal rules require a prior authorization decision within 15 days (72 hours if urgent), and complete documentation helps. For urgent medical situations, expedited reviews are available—your doctor must indicate the request is urgent. Incomplete submissions take longer due to back-and-forth for additional information.
