Does TRICARE Cover Wegovy?

Plans, Prior Auth & 2025-2026 Coverage Guide

IT DEPENDS ON YOUR PLAN

Does TRICARE Cover Wegovy?

Coverage Changed in August 2025

TRICARE coverage for Wegovy now depends on which plan you have.

  • TRICARE Prime, Select, and related plans: Still cover Wegovy with prior authorization (PA)—your doctor must get approval before you fill the prescription
  • TRICARE For Life (TFL): Lost coverage on August 31, 2025[1]

TRICARE coverage for Wegovy splits into three categories as of November 2025:

TRICARE plan coverage for Wegovy
TRICARE PlanWegovy for Weight LossStatusNotes
PrimeCovered with PAActiveStep therapy + PA required
Prime RemoteCovered with PAActiveStep therapy + PA required
SelectCovered with PAActiveStep therapy + PA required
Young AdultCovered with PAActiveStep therapy + PA required
Reserve SelectCovered with PAActiveStep therapy + PA required
Retired ReserveCovered with PAActiveStep therapy + PA required
US Family Health PlanCovered with PAActiveStep therapy + PA required
CHCBP (Continued Health Care Benefit Program)Covered with PAActiveStep therapy + PA required
TRICARE For Life (TFL)NOT CoveredEnded 8/31/25Policy exclusion; diabetes drugs still covered
Direct Care OnlyNOT CoveredEnded 8/31/25Military pharmacy access without plan enrollment

Key takeaway: If you have TRICARE Prime, Select, or related plans, Wegovy IS covered—but you'll need to navigate PA and step therapy (trying lower-cost medications first). If you have TFL, weight loss drug coverage ended August 31, 2025.

WHAT HAPPENED TO TRICARE FOR LIFE?

The August 2025 Policy Change

TFL Coverage Ended August 31, 2025

On August 31, 2025, the Defense Health Agency (DHA) implemented "regulatory controls" that ended weight loss medication coverage for TRICARE For Life beneficiaries and those with direct care only.[1][2]

What Changed

DHA's official statement: "TRICARE has been providing coverage for weight loss medications to beneficiaries who were not eligible under federal statute. This implementation corrects that unauthorized coverage."[2]

This means:

  • Even if you had an approved PA before August 31, it became invalid on that date
  • You now pay 100% out of pocket for Wegovy, Zepbound, Saxenda, Qsymia, Phentermine, and Contrave if prescribed for weight loss
  • No grandfathering—the change applied immediately to all TFL beneficiaries

What's Still Covered for TFL

Diabetes Indication Still Covered

GLP-1 medications (drugs that help control blood sugar and appetite) prescribed for Type 2 diabetes remain covered for TFL. This includes Ozempic, Mounjaro, and Victoza—with prior authorization.[1]

If you have diabetes, ask your provider if these medications might work for you.

Advocacy Response

The Military Officers Association of America (MOAA) has challenged this change, arguing it:

  • Creates "two classes of drug coverage" between TRICARE and TFL beneficiaries
  • Sets a dangerous precedent for future benefit reductions
  • Has no clear legal basis—MOAA found no law specifically excluding TFL from GLP-1 coverage[1]

The advocacy effort is ongoing, but no immediate reversal is expected. TFL beneficiaries should explore alternatives (see Options if TRICARE Won't Cover).

TFL vs. Standalone Medicare

TRICARE For Life is NOT the same as standalone Medicare. TFL is a TRICARE plan that works alongside Medicare for military retirees.

Both TFL and basic Medicare Part D coverage exclude weight-loss drugs—but they have different rules and appeals processes. If you also have Medicare Part D, you may be able to get Wegovy through the Medicare GLP-1 Bridge ($50 per monthly fill, July 1, 2026 – December 31, 2027), with conditions. See our Medicare coverage guide for details.

TRICARE PRIME & SELECT

WHAT TRICARE REQUIRES FIRST

Step Therapy Requirements

TRICARE enforces step therapy—a requirement that you try less expensive, preferred medications first before the insurance will approve more expensive drugs like Wegovy.[3]

TRICARE step therapy preferred drugs
DrugFormWhy TRICARE Requires It FirstCommon Issues
PhentermineOral tabletMore affordable, established, well-studiedInsomnia, elevated BP, anxiety
QsymiaOral capsuleCombination therapy, modest efficacyCognitive effects, increased HR
SaxendaDaily injectionGLP-1 agonist, less potent than WegovyDaily injections, GI side effects
ContraveOral tabletUnique mechanism, lower costNausea, elevated BP

Step Therapy Exception

If you've tried and failed a preferred drug in the last 180 days, or have documented contraindications to all preferred drugs, you may qualify for Wegovy without repeating step therapy.[3]

How to Document Step Therapy Failure

For your step therapy to count, your medical record must show:

  • Drug name and dates of the trial period
  • Duration (typically 3+ months adequate trial)
  • Outcome—insufficient weight loss (<5%) or intolerable side effects
  • Reason for discontinuation documented by provider

For more on navigating prior authorization and step therapy requirements, see our prior authorization guide.

CLARIFYING THE OSA INDICATION

Sleep Apnea & Wegovy

Important: Wegovy is NOT FDA-Approved for OSA

Wegovy (semaglutide) is not FDA-approved for obstructive sleep apnea. On December 20, 2024, the FDA approved Zepbound (tirzepatide)—not Wegovy—as the first medication specifically for moderate-to-severe OSA in adults with obesity.[4]

What This Means for TRICARE Coverage

Since Wegovy is not FDA-approved for OSA, TRICARE will not cover Wegovy specifically for sleep apnea treatment. The OSA pathway that bypasses step therapy applies only to Zepbound, not Wegovy.

However, if you have both obesity and sleep apnea, there are still options:

  • Pursue Wegovy for weight management—Sleep apnea is a qualifying comorbidity that can support your PA for Wegovy under the obesity indication
  • Consider Zepbound instead—If OSA is your primary concern, Zepbound has the FDA-approved OSA indication and may have different coverage requirements. See our TRICARE Zepbound guide
  • Document sleep apnea as a comorbidity—Having diagnosed OSA (ICD-10: G47.33) strengthens your medical necessity case for weight management coverage

Why Sleep Apnea Helps Your Wegovy Case

Wegovy is not approved for OSA, but having sleep apnea documented in your medical records can help your TRICARE weight-management approval because:

  • OSA qualifies as a weight-related comorbidity for the BMI ≥27 threshold
  • Weight loss may improve sleep apnea symptoms
  • It demonstrates medical necessity beyond "cosmetic" weight loss

If You Have Sleep Apnea

Make sure your sleep study results (documenting AHI score) and OSA diagnosis are in your medical records. This documentation strengthens your prior authorization request for Wegovy under the weight management indication, even though Wegovy is not approved for OSA.

TRICARE WEGOVY COPAYS

What You'll Pay

Wegovy is a non-formulary brand-name drug on TRICARE's formulary (list of covered medications). Non-formulary means it's not a preferred drug, so you'll pay higher copays. Costs vary by where you fill your prescription.[5]

YOUR OPTIONS

If TRICARE Won't Cover

If you're facing a denial or exclusion, here are your options:

1. NovoCare Self-Pay (Not Patient Assistance)

  • Wegovy is not on Novo Nordisk’s Patient Assistance Program list, so free Wegovy through patient assistance isn't an option
  • NovoCare self-pay: $349/month ($399 for Wegovy HD 7.2 mg); If you have government insurance, ask NovoCare whether you can use self-pay pricing
  • See our Wegovy cost guide for NovoCare details

2. Wegovy Savings Card

  • Novo Nordisk savings card can reduce cost to as little as $25 for commercially insured patients
  • Only works if Wegovy is covered by your insurance—does NOT work for TFL exclusions or government programs
  • See our Wegovy cost guide for savings card details

3. Diabetes-Indication Coverage

  • If you have Type 2 diabetes, ask your provider about Ozempic (same active ingredient as Wegovy—semaglutide)
  • Ozempic IS covered on TRICARE with PA for diabetes—even for TFL

4. Sleep Apnea as Comorbidity

  • If you have diagnosed OSA (AHI ≥15), use it as a comorbidity to strengthen your Wegovy PA for weight management
  • Note: Wegovy is NOT FDA-approved for OSA—only Zepbound has this indication. See our Zepbound TRICARE guide

5. Alternative GLP-1s

  • Check if Saxenda has different formulary status—it's a preferred drug on TRICARE
  • Saxenda may have lower copay and easier approval than Wegovy

6. Professional Appeal Service

  • Consider professional appeal writing services (like Honest Care) for complex denials
  • Evidence-backed appeals with clinical citations

SPECIAL RULES

Active Duty Service Members

Zero Copay Benefit

Active duty service members pay $0 copay at military pharmacies, network retail pharmacies, and TRICARE Home Delivery—both in 2025 and 2026.[5]

Key Considerations for Active Duty

  • Medical necessity required for non-formulary drugs including Wegovy
  • Military Treatment Facility (MTF) formulary restrictions: Military pharmacies generally don't stock non-formulary drugs unless medically necessary
  • Command requirements: May need to follow Military Department-specific policies for weight management programs
  • PA process: Even if coverage will not be approved, PA form must be submitted to Express Scripts first; subsequent reconsideration allowed at your MTF

Fitness standards: If weight loss is related to meeting fitness standards, work with your unit's medical team to document medical necessity appropriately.

WHAT'S CHANGING

2026 Outlook

TRICARE's coverage split (Prime/Select covered, TFL excluded) will likely stay the same through 2026. Non-formulary copays rose from $76 to $85 on January 1, 2026.

If you have TRICARE For Life and Medicare Part D, the Medicare GLP-1 Bridge may cover Wegovy for weight loss; check our Medicare guide.

TRICARE WEGOVY COVERAGE

Frequently Asked Questions

It depends on your plan. TRICARE Prime, Select, Young Adult, Reserve Select, Retired Reserve, US Family Health Plan, and CHCBP cover Wegovy with prior authorization and step therapy. TRICARE For Life and direct care only beneficiaries lost coverage on August 31, 2025.

DHA says TFL was covering weight loss drugs for people "not eligible under federal law." The change matches Medicare's weight loss drug exclusion. MOAA has challenged this decision, but no reversal is expected soon.

No—Wegovy is not FDA-approved for sleep apnea. Only Zepbound has this approval (December 2024). However, sleep apnea counts as a comorbidity that can help your Wegovy weight loss approval. If sleep apnea is your main concern, ask about Zepbound instead.

You must try and fail (or have documented contraindications to) at least one of: Phentermine, Qsymia, Saxenda, or Contrave. If you've failed a preferred drug in the last 180 days, you may get automatic approval for Wegovy.

With approved coverage: $76 copay (2025) or $85 (2026) for 30-day retail or 90-day home delivery. Active duty service members pay $0 at all points of service. Without coverage (TFL, direct care): list price $1,349/month, or NovoCare self-pay at $349/month ($399 for Wegovy HD 7.2 mg) (if you have government insurance, ask NovoCare whether you can use self-pay pricing). Wegovy is not on Novo Nordisk's patient assistance list.

Yes. You have 90 days from the denial postmark to submit a pharmacy appeal to Express Scripts. Procedural denials (missing step therapy, insufficient documentation) can be addressed by providing the missing evidence. Plan exclusion denials (TFL policy) are very difficult to overturn.

TRICARE covers Ozempic (same active ingredient as Wegovy—semaglutide) for Type 2 diabetes with prior authorization. It's NOT covered for weight loss. If you have Type 2 diabetes, this may be an option even for TFL beneficiaries.

Approximately 10 days after Express Scripts receives the complete PA form. Initial approvals are for 12 months. Renewal requires documentation of ≥5% weight loss from baseline.
