The First GLP-1 Pill for Weight Loss

What Is Oral Wegovy?

On December 22, 2025, the FDA approved oral Wegovy (semaglutide 25mg tablet)—the first oral GLP-1 approved for weight loss. This is the same molecule as injectable Wegovy, now in a daily pill form.[1]

Key Facts About Oral Wegovy

  • Same active ingredient as injectable Wegovy (semaglutide)
  • No injections required — daily pill instead of weekly injection
  • Comparable weight loss — 16.6% in clinical trials (vs 14.9% injectable)
  • Also approved for CV risk reduction — same cardiovascular benefits
  • No refrigeration needed — store at room temperature
  • Launches January 2026 — ask your doctor now to plan ahead

Why This Matters

For the first time, you can get prescription GLP-1 weight loss medication without injections. This removes a major barrier for patients who avoid needle-based treatments.

Warning: Avoid Compounded Semaglutide Pills

Only FDA-approved Wegovy contains the patented absorption technology that allows semaglutide to work as an oral medication. Compounded or "generic" semaglutide pills sold online:

  • Are not FDA-approved for weight loss
  • Lack the SNAC absorption enhancer required for oral bioavailability
  • Have unknown safety and efficacy
  • May contain incorrect dosages or contaminants

If it's not made by Novo Nordisk, it's not real Wegovy. Always get your prescription from a licensed pharmacy.

  • Choose the injection if: You prefer weekly dosing, have trouble taking pills on an empty stomach, or want the established option with longer real-world data

Medicare and weight-loss GLP-1s: the Medicare GLP-1 Bridge

Through the Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027), a temporary CMS demonstration that runs outside the Part D benefit, people with eligible Part D coverage can get Wegovy, Foundayo or the Zepbound KwikPen for weight loss for $50 a month, if they meet CMS’s eligibility criteria and their prescriber gets prior authorization.

  • Who qualifies: Age 18+ and BMI ≥35; or BMI ≥30 with HFpEF, uncontrolled high blood pressure or CKD stage 3a+; or BMI ≥27 with prediabetes, a prior heart attack or stroke, or symptomatic PAD.
  • Who doesn’t: Not eligible if you have type 2 diabetes, moderate-to-severe obstructive sleep apnea, or noncirrhotic MASH with moderate-to-advanced liver fibrosis (F2–F3), or if your Part D plan paid for a GLP-1 for you at any time in 2026.
  • Cost: $50 per monthly fill. Does not count toward the Part D deductible or out-of-pocket cap. Extra Help (LIS) does not lower it. Not eligible for the Medicare Prescription Payment Plan. Coupons and discount programs can’t be applied.
  • No opt-in needed: Part D plans and pharmacies don’t opt in, and you don’t enroll separately; your prescriber must get prior authorization.
  • If it’s denied: No appeals process for a Bridge prior-authorization denial: your prescriber can resubmit with more information (Part D appeal rights for Part D coverage decisions still apply).

Who Should Consider Oral Wegovy?

Best Candidates for Oral Wegovy

  • Needle-averse patients — Fear of injections shouldn't prevent access to effective treatment
  • Frequent travelers — No refrigeration required, easier to pack
  • Convenience preference — Daily pill may fit better into your routine than weekly injection
  • Starting treatment in 2026 — New option to discuss with your doctor
  • Cost-conscious patients — Starting dose may be around $149/month, lower than typical injectable cash prices

May Want to Stick with Injectable

  • Difficulty with empty stomach requirement — Must take 30 min before food/drink
  • Prefer weekly over daily — Once-a-week injection may be easier to remember
  • Already stable on injectable — No need to switch if current treatment is working
  • GI issues with oral medications — Some patients tolerate injections better

Ask Your Doctor

The best choice depends on your individual circumstances, preferences, and medical history. Discuss both options with your healthcare provider to determine which form is right for you.

Oral Wegovy FAQs

Frequently Asked Questions

Now. Oral Wegovy was FDA approved on December 22, 2025 and reached US pharmacies on January 5, 2026. Talk to your doctor about whether it fits you.

Yes. In the OASIS 4 trial, oral Wegovy (25mg) produced 16.6% weight loss, which is comparable to injectable Wegovy (14.9% in STEP 1). Both forms contain the same active ingredient (semaglutide).

Starting dose: $149/month through NovoCare Pharmacy (self-pay). Maintenance doses (9mg, 25mg): $299/month self-pay. With commercial insurance + savings card: May be as low as $25/month if your plan covers Wegovy. Medicare Part D enrollees who qualify for the Medicare GLP-1 Bridge pay $50 per monthly fill (July 1, 2026 – December 31, 2027). Actual costs vary by plan.

Take once daily on an empty stomach with no more than 4 oz of water. Swallow whole—do not crush, chew, or split. Wait at least 30 minutes before eating, drinking anything else, or taking other medications.

No! Unlike the injectable, oral Wegovy can be stored at room temperature. This makes it much easier for travel and daily use.

Yes, you can discuss switching with your doctor. Both contain the same active ingredient. Your doctor will determine the appropriate oral starting dose based on your current injectable dose.

Yes. Oral Wegovy is approved for both chronic weight management and to reduce the risk of major adverse cardiovascular events (MACE) in adults with established cardiovascular disease and either obesity or overweight.

Similar to injectable: nausea (47%), vomiting (31%), constipation (20%), diarrhea (18%), and abdominal pain (~15%). Most side effects are mild-to-moderate and improve over time. The main advantage: no injection-site reactions.

Coverage is expected to be similar to injectable Wegovy. Commercial insurance coverage varies by plan — check our insurance coverage hub to find your insurer. The Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027) covers Wegovy (pill and injection) for weight loss for $50 per monthly fill if you meet CMS’s criteria and your prescriber gets prior authorization. See our Medicare coverage guide for details.

Most major pharmacies (CVS, Walgreens, Walmart) stock oral Wegovy (launched January 5, 2026). You can also use mail-order pharmacies like Express Scripts or OptumRx for 90-day supplies, or NovoCare Pharmacy for self-pay pricing ($149–$299/month). Call ahead to confirm availability.

Insurance denials are common for GLP-1 medications. You have the right to appeal. Common denial reasons include step therapy requirements, BMI criteria, or formulary exclusions. See our complete appeals guide for step-by-step help, or start your free assessment to see if you qualify for our appeal support.

Talk to your doctor about transitioning. Generally, you would stop the weekly injection and start the oral pill the following week at the 1.5mg starting dose, then escalate monthly. Your doctor may adjust the transition based on your current dose and tolerability. Do not take both forms simultaneously.

No. There is no FDA-approved generic oral semaglutide for weight loss. The Wegovy pill uses patented SNAC absorption technology that allows semaglutide to be absorbed orally. "Compounded" semaglutide pills sold online lack this technology and are not proven safe or effective — avoid them.
