Pay Just $25/Fill
with commercial insurance that covers the single-dose penThe official Zepbound Savings Card (by Eli Lilly) works with your commercial insurance — not instead of it — saving you far more than GoodRx or SingleCare.
Denied coverage? You Have Options.
In one survey, nearly half of patients said their health plan didn't approve a GLP-1 the first time it was prescribed for weight loss.[6] But a successful appeal unlocks insurance coverage and the savings card (as little as $25/fill).
Over 65% of appeals succeed when patients fight back.[7] Appeals are free and take about 15 minutes to start.
OFFICIAL MANUFACTURER DISCOUNT CARD
Where to Get Your Zepbound Coupon
The official Zepbound manufacturer coupon is available directly from Eli Lilly — the company that makes Zepbound. Unlike third-party discount cards, this official coupon works alongside your insurance to maximize your savings.
Important: The "Zepbound coupon" and "Zepbound Savings Card" are the same thing — Eli Lilly calls it a savings card, but it functions like a manufacturer coupon that reduces your out-of-pocket cost. You'll receive a card with BIN, PCN, Group, and Member ID numbers to present at the pharmacy.
UNDERSTANDING THE PROGRAM
What Is the Zepbound Savings Card?
Bottom Line
If your commercial plan covers the Zepbound single-dose pen, you can pay as little as $25 per fill. The savings card is free; you sign up and activate it at zepbound.lilly.com, and you can use it at LillyDirect or at retail pharmacies.[3] For covered patients, this is the lowest price available.[1]
The Zepbound Savings Card (also called a copay card or manufacturer coupon) is a free program from Eli Lilly that lowers your copay (by up to $100 per 1-month fill) if you have commercial insurance that covers the single-dose pen. Here's what you need to know:
- Not insurance — it works alongside your existing commercial insurance
- Activation required — you must register and activate the card before use
- Annual limits apply — maximum savings of $1,300 per calendar year (2026 terms)
- Expires December 31, 2026 — Lilly hasn't published 2027 terms yet[2]
How Much Can You Save?
The Zepbound coupon (savings card) can reduce your cost to as little as $25 per fill if your commercial insurance covers the Zepbound single-dose pen and your cost before the card is $125 or less (with a higher copay, you still save up to the maximum). 2026 terms: Max savings $100/fill (1-month) or $300 (3-month), $1,300/year cap. Without coverage: $499/month (single-dose pen) or $299 / $399 / $449 a month by dose (KwikPen).[1]
| Your Situation | Your Cost | Max Savings (2026) |
|---|---|---|
| Insurance covers the single-dose pen | As low as $25/fill | $100 (1-mo), $200 (2-mo), $300 (3-mo) · $1,300/year |
| Insurance doesn't cover Zepbound | As low as $499/month (single-dose pen); KwikPen: $299 / $399 / $449 a month by dose (KwikPen) (for patients who qualify under non-covered benefit terms) | Single-dose pen: up to the the list price minus $499 per 1-month fill (Lilly states no fixed dollar cap) · KwikPen: up to 11 fills a year |
A "month" is defined as 28 days and up to 4 single-dose pens (or 1 KwikPen). Maximum 13 fills per calendar year for the single-dose pen (11 fills for the KwikPen). Card expires December 31, 2026.[1]

WHO QUALIFIES (AND WHO DOESN'T)
Savings Card Eligibility
You ARE Eligible If:
Not sure if Zepbound is covered? Check your insurer's coverage first—the savings card only reduces your copay if your plan already covers Zepbound.
- You have commercial (private) drug insurance
- You are a U.S. or Puerto Rico resident
- You are 18 years or older
- You have a valid prescription for an FDA-approved use of Zepbound[1]
You Are NOT Eligible If:
- Medicare (including Part D and Medicare Advantage)
- Medicaid
- Tricare / CHAMPUS
- VA or DoD benefits
- Any state or federal healthcare program
- Your plan uses an alternate funding program that requires you to apply for the card
- You have no insurance (cash-pay patients have other options)
If you're on Medicare or Medicaid, you cannot use this savings card — Lilly's terms exclude all government insurance (federal anti-kickback rules restrict manufacturer copay help in these programs). However, other options exist:
- Medicare patients: Ask about the Medicare GLP-1 Bridge ($50 per monthly fill for the Zepbound KwikPen, with conditions) or pay cash through LillyDirect. Note: Zepbound, Mounjaro and Foundayo are not on Lilly Cares’ medication list (see our patient assistance guide)
- No insurance: Consider LillyDirect ($299–$449/month self-pay for the KwikPen or vials)
State Restrictions
Savings card benefits may be limited in certain states:
- Massachusetts: Card not valid if an AB-rated generic equivalent is available
- California: Card not valid if an FDA-approved therapeutic equivalent is available
FDA has not approved a generic tirzepatide (Drugs@FDA lists only Lilly's own applications), so these restrictions shouldn't affect you today.[1]
STEP-BY-STEP GUIDE
How to Activate Your Zepbound Coupon
Follow these steps to activate your Zepbound coupon (savings card):
Visit the Official Website
Go to zepbound.lilly.com/coverage-savings to access the savings card enrollment page.
Enter Your Information
You'll need to provide:
- Full name and date of birth
- Address and contact information
- Insurance details (plan name, member ID)
- Prescribing doctor's information
Confirm Eligibility
Answer the eligibility questions honestly — confirming you have commercial insurance (not Medicare, Medicaid, or other government programs). You'll also need to agree to the HIPAA authorization.
Download or Print Your Card
After registration, you'll receive a digital card with your unique ID number, BIN, PCN, and Group numbers. Save this to your phone or print it for the pharmacy.
Activate the Card
Lilly's terms say card activation is required. Follow the activation instructions you get when you sign up, or call the Zepbound Savings Card program at 1-866-923-1953.[1]
Pro Tip
Complete the activation before going to the pharmacy. Some pharmacies won't process an inactive card, which means waiting while they troubleshoot.
WHAT TO SAY & DO
How to Use Your Zepbound Coupon at the Pharmacy
Once your card is activated, here's how to use it successfully at the pharmacy:
What to Bring
- Your Zepbound prescription (electronic or paper)
- Your insurance card (primary)
- Your activated Zepbound Savings Card (with BIN, PCN, Group, and ID visible)
Zepbound Savings Card Numbers
Have these numbers from your activated card ready at the pharmacy
All four numbers are printed on your activated Zepbound Savings Card.
What to Tell the Pharmacist
"I have a Zepbound prescription and I'd like to use the manufacturer savings card as a secondary payer after my insurance. Here's my insurance card and my savings card. Please process my insurance first, then apply the savings card to cover my remaining copay."
Key Points for the Pharmacist
- Process insurance first — Your commercial insurance should be billed as the primary payer
- Apply savings card second — The savings card covers your remaining out-of-pocket cost (copay/coinsurance)
- Use the BIN/PCN/Group numbers — These are on your savings card and are required for processing
- Coordination of benefits — The pharmacy system should process this as a "secondary" insurance claim
Can't Get It to Work?
Call the Zepbound Savings Card program at 1-866-923-1953 for help with the card. (The Lilly Answers Center, 1-800-545-5979, is for medical questions.)[2]
TROUBLESHOOTING ISSUES
Common Problems & Solutions
These are the most common issues patients face with the Zepbound Savings Card — and how to fix them:
"The pharmacy says the card isn't working"
Likely causes:
- Card wasn't activated before use
- Pharmacist processed it as a primary (not secondary) payer
- BIN/PCN/Group numbers entered incorrectly
Solution: Confirm the card is activated. Ask the pharmacist to re-run it as a secondary claim using coordination of benefits. If still not working, call the Zepbound Savings Card program at 1-866-923-1953 while at the pharmacy.
"I'm still being charged more than $25"
Likely causes:
- Your insurance doesn't cover Zepbound (the not-covered card prices the single-dose pen at $499/month, not $25)
- Your prescription is for the KwikPen or vials (the $25 price is for the single-dose pen only)
- You've exceeded the monthly savings cap ($100 per 1-month fill in 2026)
- You've exceeded the annual savings cap ($1,300/year in 2026)
Solution: Check your insurance coverage status. If your copay is higher than the savings cap ($100 per 1-month fill in 2026), you'll pay the difference. If you've hit annual limits, compare LillyDirect self-pay ($299–$449/month); Lilly Cares doesn't cover Zepbound (see our patient assistance guide).
"Prior authorization required"
What's happening: Your insurance requires pre-approval before covering Zepbound. The savings card can't help until your insurance approves the medication.
Solution: Contact your prescriber to submit the prior authorization paperwork. How long the plan takes varies, so ask the office to tell you when it's approved. Once approved, return to the pharmacy with your savings card. See our prior authorization guide for PA tips.
"Card rejected — government insurance detected"
What's happening: The system detected that you have Medicare, Medicaid, Tricare, or another government program. These patients are not eligible for the savings card.
Solution: If you genuinely have only commercial insurance, call the Zepbound Savings Card program to resolve the error. If you do have government insurance, see our patient assistance guide: on Medicare, the GLP-1 Bridge covers the Zepbound KwikPen if you qualify, and Lilly Cares doesn't cover Zepbound.
"The pharmacy has never heard of this card"
What's happening: Some smaller or independent pharmacies may be unfamiliar with manufacturer copay cards.
Solution: Explain that it's a manufacturer copay assistance card (not a discount coupon). Provide the BIN, PCN, Group, and ID numbers. The pharmacy should process it like a secondary insurance. If they refuse or can't figure it out, try a larger chain pharmacy (such as CVS or Walgreens) that regularly handles these.
CHOOSING THE RIGHT OPTION
Savings Card vs. Other Discount Options
The Zepbound Savings Card isn't the only way to save. Here's how it compares to other options:
| Option | Best For | Potential Savings | Key Limitation |
|---|---|---|---|
| Zepbound Savings Card | Commercial insurance (covered) | As low as $25/fill | No government insurance |
| LillyDirect | No insurance / not covered | $299–$449/month (self-pay KwikPen or vials) | Cash only, bypasses insurance |
| Patient Assistance (PAP) | Low-income patients | Not available for Zepbound | Zepbound, Mounjaro and Foundayo are not on Lilly Cares’ medication list |
| GoodRx / SingleCare | Cash-pay patients | Varies by pharmacy; check the current price | Can't combine with insurance |
Which Should You Use?
If your insurance covers Zepbound
Use the Zepbound Savings Card. Lilly covers up to $100 per 1-month fill of your copay (2026 terms), so you pay as little as $25 per fill when your cost before the card is $125 or less; with a higher copay, you pay your cost minus the saving.
If your insurance doesn't cover Zepbound
Consider LillyDirect. The 2026 savings card brings the single-dose pen to $499/month and prices the KwikPen at $299 / $399 / $449 a month by dose (KwikPen); LillyDirect self-pay for the KwikPen or vials is $299–$449/month. You can also appeal the coverage denial.
If you have Medicare or Medicaid
Check the Medicare GLP-1 Bridge. The savings card's terms exclude government insurance, and Zepbound, Mounjaro and Foundayo are not on Lilly Cares’ medication list. On Medicare, the Bridge covers the Zepbound KwikPen at $50 per monthly fill if you qualify; you can also pay cash through LillyDirect. See our patient assistance guide.
For a complete breakdown of all Zepbound pricing options, see our Zepbound cost overview.
SAVINGS CARD & MEDICARE CHANGES
What's Coming in 2026
2026 Will Be a Big Year for Zepbound Affordability
Major changes in 2026: the Medicare GLP-1 Bridge, new direct-purchase options, and the renewed savings card. Here's what we know — and what's still TBD.
Confirmed for 2026
Medicare GLP-1 Bridge Program — July 1, 2026
How the Medicare GLP-1 Bridge works for Zepbound
Through the Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027), a temporary CMS demonstration that runs outside the Part D benefit, people with eligible Part D coverage can get Wegovy, Foundayo or the Zepbound KwikPen for weight loss for $50 a month, if they meet CMS’s eligibility criteria and their prescriber gets prior authorization.
- Zepbound: Zepbound KwikPen only (not vials or single-dose pens).
- Who qualifies: Age 18+ and BMI ≥35; or BMI ≥30 with HFpEF, uncontrolled high blood pressure or CKD stage 3a+; or BMI ≥27 with prediabetes, a prior heart attack or stroke, or symptomatic PAD.
- Who doesn’t: Not eligible if you have type 2 diabetes, moderate-to-severe obstructive sleep apnea, or noncirrhotic MASH with moderate-to-advanced liver fibrosis (F2–F3), or if your Part D plan paid for a GLP-1 for you at any time in 2026.
- Cost: $50 per monthly fill. Does not count toward the Part D deductible or out-of-pocket cap. Extra Help (LIS) does not lower it. Not eligible for the Medicare Prescription Payment Plan. Coupons and discount programs can’t be applied.
- No opt-in needed: Part D plans and pharmacies don’t opt in, and you don’t enroll separately; your prescriber must get prior authorization.
- If it’s denied: No appeals process for a Bridge prior-authorization denial: your prescriber can resubmit with more information (Part D appeal rights for Part D coverage decisions still apply).

What this means: If you're on Medicare and have been paying out-of-pocket for Zepbound, you may now get coverage for the KwikPen through the Bridge, a temporary CMS program that runs outside the Part D benefit (prior authorization required). Outside the Bridge: Basic Medicare Part D coverage excludes drugs used for weight loss.[4]
TrumpRx Direct-Purchase Platform — NOW LIVE
TrumpRx.gov launched February 5, 2026. TrumpRx.gov lists Lilly's own self-pay prices (orders go through LillyDirect):[5]
- Price: the same as LillyDirect self-pay ($299–$449/month; the Zepbound listing is the vial)
- Who can use it: Self-pay patients with a prescription (eligibility and Lilly's terms apply)
- No prior authorization required
2026 Savings Card — Now Available
2026 Savings Card Terms — Updated
The Zepbound Savings Card has been renewed for 2026 (expires December 31, 2026). 2026 terms:
- With coverage: As low as $25/fill (when your cost before the card is $125 or less), $100 per 1-month fill max savings, $1,300/year cap
- Without coverage: As low as $499/month for the single-dose pen (KwikPen: $299 / $399 / $449 a month by dose (KwikPen)), with a maximum saving on the single-dose pen of the list price minus $499 per 1-month fill (Lilly states no fixed dollar cap)
Tip: For higher doses without coverage, LillyDirect self-pay (KwikPen or vials) ($449/mo if you refill within 45 days) may be cheaper than the single-dose pen with the savings card ($499/month). Compare at zepbound.lilly.com.
Medicaid Coverage Expansion
Medicaid's BALANCE Model: Voluntary for states, which can join between May 2026 and January 1, 2027; no participating state has been confirmed. Medicaid coverage of Zepbound for weight loss depends on your state—always verify with your state Medicaid program. The savings card cannot be used with Medicaid (government insurance).[4]
What Should You Do Now?
The 2026 savings card is available now. Visit zepbound.lilly.com to enroll or renew. Card valid through December 31, 2026.
The Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027) covers the Zepbound KwikPen at $50 per monthly fill if you qualify. You can also pay cash through LillyDirect; TrumpRx lists the same Lilly self-pay prices.
Try LillyDirect ($299–$449/month); TrumpRx lists the same Lilly self-pay prices (orders go through LillyDirect). New: Foundayo (oral GLP-1) at $149–$299/month self-pay.
COMMON QUESTIONS ABOUT THE SAVINGS CARD
Frequently Asked Questions
The official Zepbound coupon is available free at zepbound.lilly.com/coverage-savings. This manufacturer coupon (also called the Zepbound Savings Card) can reduce your cost to as little as $25 per fill if you have commercial insurance. Unlike third-party discount cards (GoodRx, SingleCare), the official Zepbound coupon works with your insurance, not instead of it — providing much larger savings for brand-name medications.
No, the Zepbound coupon doesn't use a promo code. Instead, you'll receive a physical or digital card with BIN/PCN/Group/Member ID numbers to present at the pharmacy. There's no code to enter online — the savings are applied at checkout when your pharmacist processes the card as secondary insurance. Get your card free at zepbound.lilly.com/coverage-savings.
If your commercial insurance covers the Zepbound single-dose pen, you can pay as little as $25 per fill (1, 2, or 3 months). If your insurance doesn't cover Zepbound, the 2026 savings card brings the single-dose pen to as low as $499/month (maximum saving: the list price minus $499 per 1-month fill (Lilly states no fixed dollar cap)); the KwikPen is $299 / $399 / $449 a month by dose (KwikPen).
No. The card's terms exclude Medicare (including Part D and Medicare Advantage), Medicaid, TRICARE, VA, DoD and other government programs, in line with federal anti-kickback rules. If you have Medicare, the Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027) covers the Zepbound KwikPen at $50 per monthly fill if you qualify, and you can pay cash through LillyDirect. Zepbound, Mounjaro and Foundayo are not on Lilly Cares’ medication list; see our patient assistance guide.
The current savings card program expires December 31, 2026. Lilly hasn't published 2027 terms yet. Visit zepbound.lilly.com for the latest information.
Common issues include: (1) Card not activated, (2) Pharmacy processing it as primary instead of secondary payer, (3) Prior authorization not yet approved by your insurance, (4) Government insurance detected in the system. Call the Zepbound Savings Card program at 1-866-923-1953 for help.
The savings card has monthly and annual limits. 2026 terms (with coverage): $100/fill (1-month), $200 (2-month), or $300 (3-month). Annual cap: $1,300. Maximum 13 fills per year. Without coverage (single-dose pen): up to the list price minus $499 per 1-month fill (Lilly states no fixed dollar cap). If your copay exceeds the cap, you pay the difference.
Not exactly. Manufacturer copay cards (like the Zepbound Savings Card) work alongside your insurance — they cover your out-of-pocket costs after insurance pays. Generic discount coupons (like GoodRx) are cash-price discounts that replace insurance and typically save less for brand-name drugs. For Zepbound, the savings card provides significantly better savings.
Generally, yes. Lilly lists Zepbound as HSA/FSA eligible, but eligibility can vary by plan, so check with your benefits administrator. You can use HSA/FSA money for what you still owe after the card is applied; the card's terms don't allow claiming the card's savings themselves from an HSA or FSA.
It can, mainly through your plan. 2026 terms: the card's maximum scales with the supply ($300 for a 3-month fill, the same as $100 a month), so a 90-day fill doesn't raise the card's monthly maximum. Some insurers have lower copays for 90-day prescriptions through mail-order pharmacies. Ask your doctor to write a 90-day prescription if you're stable on your dose.
Compounded tirzepatide (the active ingredient in Zepbound) is not FDA-approved. FDA says unapproved versions don't undergo its review for safety, effectiveness and quality, and that compounded drugs should only be used when an FDA-approved drug can't meet a patient's medical needs. The savings card only works on Lilly's own Zepbound products. LillyDirect offers the Zepbound KwikPen or vials at $299–$449/month as a legitimate alternative.
Partially. If your insurance doesn't cover Zepbound, the 2026 card brings the single-dose pen to as low as $499/month. The card prices the KwikPen at $299 / $399 / $449 a month by dose (KwikPen). Compare: LillyDirect self-pay (KwikPen or vials) costs $449/month for higher doses if you refill within 45 days.
Generally, yes. Lilly applies savings automatically at LillyDirect, and the card can be used at retail pharmacies such as CVS, Walgreens, Walmart and independents. Some mail-order pharmacies also accept it. If a pharmacy claims they don't accept it, ask them to run it as a secondary payer using the BIN/PCN/Group numbers.
The 2026 savings card is now available. Visit zepbound.lilly.com to enroll or renew. Lilly hasn't published 2027 terms yet, and its terms say you may need a new card when terms change. Card valid through December 31, 2026.
This happens when your insurance copay exceeds the card's monthly savings cap (2026 terms: $100 for 1-month supply). For example, if your copay is $150, the card covers $100, leaving you to pay $50. Solutions: (1) Ask whether your plan charges less for a 90-day supply (the card's cap is $300 for 3 months, the same per month), (2) Check if your insurer has a preferred pharmacy with lower copays, (3) Appeal for a tier exception to move Zepbound to a lower copay tier.
