IMPORTANT CLARIFICATION
Does Lilly Cares Cover Zepbound?
Zepbound Is Not Currently Covered by Lilly Cares
Lilly Cares, Eli Lilly's patient assistance program (PAP) that provides free medications to low-income patients, does not currently include Zepbound on its medication list.[1] Zepbound, Mounjaro and Foundayo are not on Lilly Cares’ medication list, so Zepbound is not available through Lilly Cares.
If you've seen information online suggesting Zepbound is covered by Lilly Cares at 300% FPL, that information is inaccurate. We've verified this directly with current program documentation.
What Is Lilly Cares?
Lilly Cares is Eli Lilly's charitable foundation that provides free medications to qualifying patients who:
- Are Uninsured or on Medicare Part D/Part B, with income limits of 300–500% of the federal poverty level by drug
- Have a valid prescription for a medicine on Lilly Cares' list
Lilly Cares covers many Lilly medications, but Zepbound, Mounjaro and Foundayo are not on Lilly Cares’ medication list.[1]
Will Lilly Cares Add Zepbound in the Future?
Lilly hasn't announced any plan to add Zepbound. To check the current list:
- Visit the official Lilly Cares website
- Call Lilly Cares at 1-800-545-6962
In the meantime, there ARE real options to make Zepbound more affordable—see below.
NOT A PAP, BUT A GREAT ALTERNATIVE
LillyDirect Self-Pay Option
For Uninsured Patients: LillyDirect
LillyDirect isn't a patient assistance program—it's Eli Lilly's direct-to-consumer pharmacy offering the Zepbound KwikPen or single-dose vials at significantly reduced cash prices:
- Starting dose (2.5 mg): $299/month
- 5 mg: $399/month
- Maintenance doses (7.5-15 mg): $449/month if you refill within 45 days (late refills: $499 (7.5 mg) or $699 (10–15 mg) if you refill after 45 days)
No income requirements. No insurance needed. Now available for pickup at Walmart pharmacies nationwide.[4]
This represents over 50% savings compared to the list price (about $1,086/month)—and unlike traditional patient assistance, any adult in the U.S. or Puerto Rico with a valid prescription can use it.[3]

FOR COMMERCIALLY INSURED PATIENTS
Zepbound Savings Card
If you have commercial (private) insurance that covers the Zepbound single-dose pen, the manufacturer savings card can reduce your cost to as little as $25 per fill.[5]
Savings Card Quick Facts
- Cost: As low as $25/month
- Max savings: $100 per 1-month fill (2026 terms)
- Annual limit: $1,300 a year (13 fills) (2026 terms)
- Plan doesn't cover it? The card prices the single-dose pen at $499/month and the KwikPen at $299 / $399 / $449 a month by dose (KwikPen)
- Expires: December 31, 2026
Who Qualifies for the Savings Card?
- Patients with commercial insurance (the $25 price needs a plan that covers the single-dose pen)
- Must have a valid prescription for an FDA-approved use
- U.S. or Puerto Rico residents 18 years or older
Who Does NOT Qualify
- Medicare (including Part D)
- Medicaid
- TRICARE, VA, or other government insurance
- Uninsured patients (use LillyDirect self-pay instead)
ADDITIONAL WAYS TO REDUCE COSTS
Other Affordability Options
Appeal Your Insurance Denial
If your insurance denied Zepbound coverage, don't give up. Over 65% of prior authorization appeals succeed.[6] An appeal can get your insurance to cover Zepbound, potentially reducing your cost to just a copay.

Independent Patient Assistance Foundations
Some nonprofit foundations provide financial assistance for medications independent of manufacturers:
- NeedyMeds — Database of assistance programs: needymeds.org
- Patient Advocate Foundation — Copay relief programs: patientadvocate.org
- RxAssist — Comprehensive database: rxassist.org
Note: Availability for GLP-1 weight loss medications varies. Check each program for current coverage.
Check State Medicaid Expansion
Only some state Medicaid programs cover GLP-1 medications for weight loss (13 states (as of January 2026)), and several states have cut coverage recently. Coverage varies significantly by state and is subject to change.
See our Medicaid coverage guide →
Consider Alternative GLP-1 Medications
If Zepbound remains unaffordable, other GLP-1 medications may have different coverage or assistance programs:
- Foundayo — Oral GLP-1 (approved April 1, 2026): $149–$299/month self-pay, as low as $25/month with savings card (commercial coverage). No injection required.
- Wegovy — Novo Nordisk has a savings offer for commercially insured patients and self-pay pricing, but Wegovy is not on Novo Nordisk’s Patient Assistance Program list
- Saxenda — May have different insurance coverage

WHAT'S CHANGING
Patient Assistance Outlook for 2026
Major Affordability Changes in 2026
While Lilly Cares still doesn't cover Zepbound, 2026 brings significant new options for patients who can't afford the medication—including TrumpRx.gov and the Medicare GLP-1 Bridge.
Confirmed for 2026
TrumpRx Direct-Purchase Program — NOW LIVE
TrumpRx.gov launched February 5, 2026. TrumpRx.gov lists Lilly's own self-pay prices (orders go through LillyDirect): $299–$449/month for Zepbound (vial listing)—no insurance or income requirements.[11]
Medicare GLP-1 Bridge Program — Live Now
How the Medicare GLP-1 Bridge works for Zepbound
Through the Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027), a temporary CMS demonstration that runs outside the Part D benefit, people with eligible Part D coverage can get Wegovy, Foundayo or the Zepbound KwikPen for weight loss for $50 a month, if they meet CMS’s eligibility criteria and their prescriber gets prior authorization.
- Zepbound: Zepbound KwikPen only (not vials or single-dose pens).
- Who qualifies: Age 18+ and BMI ≥35; or BMI ≥30 with HFpEF, uncontrolled high blood pressure or CKD stage 3a+; or BMI ≥27 with prediabetes, a prior heart attack or stroke, or symptomatic PAD.
- Who doesn’t: Not eligible if you have type 2 diabetes, moderate-to-severe obstructive sleep apnea, or noncirrhotic MASH with moderate-to-advanced liver fibrosis (F2–F3), or if your Part D plan paid for a GLP-1 for you at any time in 2026.
- Cost: $50 per monthly fill. Does not count toward the Part D deductible or out-of-pocket cap. Extra Help (LIS) does not lower it. Not eligible for the Medicare Prescription Payment Plan. Coupons and discount programs can’t be applied.
- No opt-in needed: Part D plans and pharmacies don’t opt in, and you don’t enroll separately; your prescriber must get prior authorization.
- If it’s denied: No appeals process for a Bridge prior-authorization denial: your prescriber can resubmit with more information (Part D appeal rights for Part D coverage decisions still apply).

ZEPBOUND AFFORDABILITY FAQS
Frequently Asked Questions
Not through Lilly Cares. While Eli Lilly's Lilly Cares patient assistance program provides free medications to qualifying low-income patients, Zepbound is not currently on the covered medication list. However, uninsured patients can use LillyDirect to get Zepbound at $299–$449/month (50%+ off list price), and commercially insured patients may qualify for the Zepbound Savings Card (as little as $25/month).
Currently, there is no program offering free Zepbound. The lowest-cost options are: (1) Zepbound Savings Card for as little as $25/month if you have commercial insurance that covers it, or (2) LillyDirect Self-Pay at $299/month for the starting dose if you're uninsured. Some independent nonprofit foundations may offer limited assistance—check NeedyMeds.org for current programs.
Eli Lilly has not publicly stated why Zepbound is excluded from Lilly Cares. Zepbound, Mounjaro and Foundayo are not on Lilly Cares’ medication list. Check the official Lilly Cares website for the current list.
Lilly's lowest cash price for Zepbound is through LillyDirect: $299/month for 2.5mg, $399/month for 5mg, or $449/month for maintenance doses (7.5-15mg). That is more than 50% off the list price (about $1,086/month). You can also pick up at Walmart pharmacies at the same price. TrumpRx lists the same Lilly self-pay prices (orders go through LillyDirect).
Only if you have commercial insurance that covers Zepbound. The Zepbound Savings Card reduces your copay to as low as $25/month, but that price requires: (1) commercial/private insurance, (2) a plan that covers Zepbound, and (3) no Medicare, Medicaid, or other government insurance. If your commercial plan doesn't cover Zepbound, the card still applies: $499/month for the single-dose pen (KwikPen: $299 / $399 / $449 a month by dose (KwikPen)). The 2026 savings card expires December 31, 2026.
Yes. You can use LillyDirect ($299–$449/month) to start treatment while their prior authorization or appeal is processing. If your insurance later approves coverage, you can switch to using insurance + the savings card (as little as $25/month). This ensures you don't delay treatment while navigating the insurance process.
There are several ways to make Zepbound more affordable: (1) With commercial insurance: Use the Zepbound Savings Card to pay as low as $25/month. (2) Without insurance: Use LillyDirect self-pay (KwikPen or vials) at $299–$449/month (50%+ off list price). (3) If denied by insurance: Appeal the denial—over 65% of appeals succeed. (4) Check nonprofits: Organizations like NeedyMeds and Patient Advocate Foundation may offer additional assistance.
