WITHOUT INSURANCE
Zepbound List Price
Without insurance or discounts, Zepbound costs about $1,086/month (per 28-day supply)—the same price regardless of dose strength.[1] This is Eli Lilly's official price before any discounts are applied.
| Dose | Supply | List Price | Annual Cost |
|---|---|---|---|
| 2.5 mg (starter) | 4 pens | about $1,086/month | over $14,000 |
| 5 mg | 4 pens | about $1,086/month | over $14,000 |
| 7.5 mg | 4 pens | about $1,086/month | over $14,000 |
| 10 mg | 4 pens | about $1,086/month | over $14,000 |
| 12.5 mg | 4 pens | about $1,086/month | over $14,000 |
| 15 mg | 4 pens | about $1,086/month | over $14,000 |
Important: Pharmacy Markup
Retail pharmacies add markup to the list price. Without discounts, you might see prices at or above the list price (about $1,086/month) depending on the pharmacy. Always compare prices or use discount programs.
Why the Same Price for All Doses?
The single-dose pen's list price is the same for every dose, so your list-price cost doesn't change as your doctor increases your dose. Self-pay KwikPen and vial prices are different: they rise with the dose ($299–$449/month).
WHAT YOU'LL ACTUALLY PAY
Zepbound Cost With Insurance
If your insurance covers Zepbound, your out-of-pocket cost depends on where your plan places Zepbound on its drug list (called a "formulary"), your deductible, and whether you pay a flat copay or a percentage of the cost (coinsurance). Here's what to expect for your Zepbound out-of-pocket cost:
| Insurance Scenario | Typical Monthly Cost | Notes |
|---|---|---|
| Preferred tier + Savings Card (single-dose pen) | $25 | Best case scenario |
| Non-preferred tier + Savings Card | $25 or more | Card saves up to $100 per 1-month fill (2026 terms); if your cost before the card is above $125, you still save up to that amount (you pay your cost minus the saving) |
| Covered, no savings card | Your plan's copay or coinsurance | Varies widely by plan and tier |
| Covered after deductible | Your plan's price (can be near list, about $1,086/month) → then copay | Until the deductible is met; the card can still take off up to $100 per 1-month fill |
| Not covered / excluded | Savings card: $499/month (single-dose pen) or $299 / $399 / $449 a month by dose (KwikPen) | Or LillyDirect self-pay; consider an appeal |
Coverage Varies Widely by Insurer
Insurance coverage for GLP-1 weight loss medications is inconsistent. Some plans cover Zepbound after your doctor gets pre-approval (called "prior authorization"), others exclude it entirely, and many changed policies in 2025:
- Aetna: Varies by plan—check your plan's formulary and prior authorization rules[2]
- BCBS: Varies by state and plan—check your specific policy
- Cigna: Coverage varies; Cigna plans may require step therapy; check your plan’s policy
- UnitedHealthcare: Weight-loss coverage Depends on your employer’s plan; sleep apnea has a separate Zepbound policy[3]
- CVS Caremark: Since October 1, 2026, Zepbound is back on CVS Caremark’s commercial template formularies as an additional preferred option (plan sponsors can customize) (from July 2025 to September 2026 it was nonpreferred or excluded on some of its most common commercial formularies)[4]
Check Your Coverage First
Before assuming you'll pay list price, verify your plan's formulary and prior authorization requirements. Even if initially denied, over 65% of prior authorization appeals succeed.

Uninsured with limited income?

SELF-PAY OPTIONS
Zepbound Cost Without Insurance
Before Paying Cash: Consider Appealing First
65%+ of insurance appeals succeed with proper documentation. If you were denied coverage, appealing may get you $25/month pricing instead of $299–$449/month self-pay. For a standard internal appeal, plans generally must decide within 30 days for care you haven't received yet—LillyDirect can bridge the gap while you wait.
Eli Lilly's LillyDirect offers Zepbound (KwikPen or single-dose vials) at a reduced cash price for self-pay patients. It's a good option while you wait for an appeal decision—if approved, you may be able to switch to as-little-as-$25 pricing on the single-dose pen and save thousands a year.[5]
✅ Price Drop: December 1, 2025
Eli Lilly reduced LillyDirect vial prices effective December 1, 2025: the 2.5 mg starter dose dropped from $349 to $299/month, and 5 mg and higher doses dropped from $499 to $399–$449/month (7.5 mg+ when you refill within 45 days).[11]
| Dose | LillyDirect Price | Savings vs. List | Notes |
|---|---|---|---|
| 2.5 mg (starter) | $299/month | Save $787.37 (72%) | First 4 weeks |
| 5 mg | $399/month | Save $687.37 (63%) | Weeks 5-8 |
| 7.5 mg – 15 mg | $449/month | Save $637.37 (59%) | Maintenance doses |
LillyDirect pricing updated December 1, 2025 per Eli Lilly press release; current prices checked on LillyDirect.[10]
Now Available at Walmart
Since November 2025, LillyDirect self-pay Zepbound has been available for free pickup at Walmart pharmacies (nearly 4,600 nationwide) at the same self-pay price; Lilly now lists Walmart pickup for both the vials and the KwikPen.[6]
How LillyDirect Works
- Get a prescription from your doctor (or through independent telehealth providers LillyDirect can connect you with)
- Have it sent to LillyDirect Pharmacy; a dispensing pharmacy contacts you to confirm details and checkout
- Choose delivery or pickup—now available at Walmart pharmacies nationwide[6]
- Receive the KwikPen or single-dose vials (the KwikPen needs pen needles and vials need needles and syringes, sold separately)
KwikPen or Vials, Not the Single-Dose Pen
LillyDirect's self-pay prices are for the Zepbound KwikPen (Zepbound KwikPen (four doses per pen)) or single-dose vials, not the single-dose pen. With vials, you draw and inject each dose using a syringe.
Already using LillyDirect? You can still appeal.
If you were denied coverage, you can use LillyDirect now while appealing. If your appeal succeeds, switch to insurance + savings card pricing ($25/month) and save thousands per year.

WHAT'S CHANGING
Zepbound Cost Outlook for 2026
Major Pricing Changes in 2026
2026 brought significant changes to Zepbound affordability—including TrumpRx.gov (a federal site listing manufacturers' self-pay prices) and Medicare coverage through the GLP-1 Bridge. Here's where things stand.
Confirmed for 2026
TrumpRx Direct-Purchase Program — NOW LIVE
TrumpRx.gov launched February 5, 2026. TrumpRx.gov lists Lilly's own self-pay prices (orders go through LillyDirect); its Zepbound listing is the vial, at $299–$449/month—no insurance required.[10]
Medicare GLP-1 Bridge Program — July 1, 2026
The Medicare GLP-1 Bridge runs July 1, 2026 – December 31, 2027 and covers the Zepbound KwikPen only (not vials or single-dose pens) at $50 per monthly fill. Who qualifies: Age 18+ and BMI ≥35; or BMI ≥30 with HFpEF, uncontrolled high blood pressure or CKD stage 3a+; or BMI ≥27 with prediabetes, a prior heart attack or stroke, or symptomatic PAD. Who doesn't: Not eligible if you have type 2 diabetes, moderate-to-severe obstructive sleep apnea, or noncirrhotic MASH with moderate-to-advanced liver fibrosis (F2–F3), or if your Part D plan paid for a GLP-1 for you at any time in 2026. Part D plans and pharmacies don’t opt in, and you don’t enroll separately; your prescriber must get prior authorization.[11]
LillyDirect Continues
LillyDirect self-pay for the KwikPen or vials ($299–$449/month) remains available. TrumpRx.gov lists Lilly's own self-pay prices (orders go through LillyDirect).
Foundayo (Oral GLP-1) — NEW April 2026
Foundayo (orforglipron) was FDA-approved April 1, 2026—the only GLP-1 pill for weight loss that can be taken any time of day without food or water restrictions (Lilly). Self-pay: $149–$299/month. With savings card (if your commercial plan covers it): as low as $25/month. No injections, stored at room temperature, taken once daily with or without food.
Pending / TBD
Savings Card Renewed for 2026 — Terms Changed
The Zepbound Savings Card has been renewed through December 31, 2026. 2026 terms: with coverage: still as little as $25/month (when your cost before the card is $125 or less), but the maximum saving is $100 per 1-month fill ($1,300 a year (13 fills)). Without coverage: $499/month for the single-dose pen (maximum saving: the list price minus $499 per 1-month fill (Lilly states no fixed dollar cap)), or $299 / $399 / $449 a month by dose (KwikPen).
Lilly Cares PAP Expansion
Zepbound, Mounjaro and Foundayo are not on Lilly Cares’ medication list, and Lilly hasn't announced plans to add Zepbound. Without coverage, LillyDirect self-pay ($299–$449/month) is the main option; TrumpRx.gov lists the same Lilly prices (orders go through LillyDirect).
More Insurers Dropping Coverage
Following CVS Caremark's 2025 formulary change (Zepbound nonpreferred or excluded on some of its most common commercial formularies; since reversed: Since October 1, 2026, Zepbound is back on CVS Caremark’s commercial template formularies as an additional preferred option (plan sponsors can customize)) and BCBS Massachusetts (January 2026), more PBMs and insurers may restrict GLP-1 coverage due to cost pressures. This makes direct-purchase options increasingly important.
2026 Cost Projections
| Payment Method | 2025 Cost | 2026 Projected |
|---|---|---|
| TrumpRx | N/A | Lilly's own self-pay prices (live now) |
| Medicare Part D | Not covered for weight loss alone (sleep apnea use could be) | Bridge: $50 per monthly fill (with conditions; KwikPen only) |
| LillyDirect | $299–$449/month | Same or similar |
| Savings Card | As little as $25/mo | Renewed through Dec 2026 |
| Foundayo (oral) | N/A | $149–$299/month self-pay, $25/mo w/card |
What Should You Do Now?
- Using the Savings Card? Good news—renewed through December 31, 2026
- On Medicare? The Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027) covers the Zepbound KwikPen at $50 per monthly fill for eligible patients; you can also pay cash through LillyDirect
- Uninsured? LillyDirect self-pay is $299–$449/month; TrumpRx.gov lists the same Lilly prices (orders go through LillyDirect)
- Prefer pills over injections? Foundayo (oral GLP-1) is now FDA-approved at $149–$299/month self-pay
- Insurance dropping coverage? Budget for direct-purchase alternatives
