Coverage Options
Foundayo Cost With Insurance
If your commercial insurance covers Foundayo, you can pay as little as $25/month using the free Foundayo Savings Card. However, because Foundayo was just FDA-approved on April 1, 2026, coverage policies are still forming at most insurers.[1]
| Insurance Scenario | Monthly Cost | Notes |
|---|---|---|
| Covered + Savings Card | $25/month | Best case scenario |
| Covered, no savings card | Varies by plan | Depends on tier and copay structure |
| Not covered | the same as self-pay pricing ($149–$299/month), up to 10 fills | Savings card (commercial insurance) or LillyDirect self-pay |
What to Expect as a New Drug
As a brand new medication, Foundayo faces the same coverage challenges as other GLP-1 weight loss drugs:
- Prior authorization likely required: Most insurers require your doctor to submit documentation before approving coverage
- Coverage policies still forming: CVS Caremark ended its new-to-market block on Foundayo on June 1, 2026, where plans have approved it for coverage; other PBMs' (pharmacy benefit managers') decisions vary by plan
- Appeals often succeed: If initially denied, 65%+ of GLP-1 appeals succeed with proper documentation[2]
Don't Assume You're Not Covered
Even if your insurer hasn't published a formal policy on Foundayo yet, they may still approve coverage on a case-by-case basis. Ask your doctor to submit for prior authorization and see what happens. If denied, we can help you appeal.

Cash Pay Options
Foundayo Cost Without Insurance (Self-Pay)
If you don't have insurance or your plan doesn't cover Foundayo, LillyDirect offers direct-to-consumer pricing at $149–$299/month depending on your dose. This is far below injectable GLP-1 list prices, which can be $1,000+ per month without insurance.[1]
LillyDirect: $149-$299/Month
Eli Lilly's direct-to-consumer platform offers Foundayo pills at reduced cash prices:
- Starting doses (0.8mg and 2.5mg): $149/month
- Titration dose (5.5mg): $199/month
- Maintenance doses (9mg+): $299/month
Free home delivery included. Shipping began April 6, 2026.
Why Oral Foundayo Is More Affordable
Foundayo's oral pill format offers genuine cost advantages over injectable GLP-1s:
- Lower manufacturing costs: Pills are cheaper to produce than pre-filled injection pens
- No injection supplies: No syringes, alcohol swabs, or sharps containers to buy
- No refrigeration: Store at room temperature (no cold chain logistics)
- Easier distribution: Ships like any other medication
Consider Appealing First
If you were denied insurance coverage, appeal before settling for self-pay. 65%+ of GLP-1 appeals succeed with proper documentation.[2] $25/month (with coverage) vs $149–$299/month (self-pay) is a significant difference over a year.

Dose-by-Dose Breakdown
Foundayo Pricing by Dose
Foundayo comes in six commercial doses. Your doctor will start you at the lowest dose (0.8mg) and gradually increase based on your response and tolerability. Through LillyDirect, pricing varies by dose tier.[1]
| Dose | LillyDirect Price | Notes |
|---|---|---|
| 0.8mg | $149/month | Starting dose |
| 2.5mg | $149/month | Titration |
| 5.5mg | $199/month | Titration |
| 9mg | $299/month | Titration |
| 14.5mg | $299/month | Maintenance |
| 17.2mg | $299/month | Highest dose (max efficacy) |
| Lapsed refill (>45 days, 14.5mg/17.2mg) | $349/month | Refill penalty |
45-Day Refill Window
To keep the $299/month price on the 14.5mg and 17.2mg doses, refill your prescription within 45 days of your last fill. If you go longer than 45 days without refilling, the price increases to $349/month. The 2.5mg and 5.5mg prices are temporarily reduced through December 31, 2026. Set a reminder so you don't miss the window.
Commercial vs. Clinical Trial Doses
Important: The ATTAIN clinical trials used different doses (6mg, 12mg, 36mg) than the commercial product. The commercial doses (0.8mg through 17.2mg) are formulated for gradual titration. Your doctor will guide you through the commercial dose schedule.[3]

Medicare GLP-1 Bridge
Medicare Coverage for Foundayo
Medicare and weight-loss GLP-1s: the Medicare GLP-1 Bridge
Through the Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027), a temporary CMS demonstration that runs outside the Part D benefit, people with eligible Part D coverage can get Wegovy, Foundayo or the Zepbound KwikPen for weight loss for $50 a month, if they meet CMS’s eligibility criteria and their prescriber gets prior authorization.
- Who qualifies: Age 18+ and BMI ≥35; or BMI ≥30 with HFpEF, uncontrolled high blood pressure or CKD stage 3a+; or BMI ≥27 with prediabetes, a prior heart attack or stroke, or symptomatic PAD.
- Who doesn’t: Not eligible if you have type 2 diabetes, moderate-to-severe obstructive sleep apnea, or noncirrhotic MASH with moderate-to-advanced liver fibrosis (F2–F3), or if your Part D plan paid for a GLP-1 for you at any time in 2026.
- Cost: $50 per monthly fill. Does not count toward the Part D deductible or out-of-pocket cap. Extra Help (LIS) does not lower it. Not eligible for the Medicare Prescription Payment Plan. Coupons and discount programs can’t be applied.
- No opt-in needed: Part D plans and pharmacies don’t opt in, and you don’t enroll separately; your prescriber must get prior authorization.
- If it’s denied: No appeals process for a Bridge prior-authorization denial: your prescriber can resubmit with more information (Part D appeal rights for Part D coverage decisions still apply).

What Medicare Patients Can Do Now
- Ask your doctor about the Bridge: After the pharmacy sends the claim, your prescriber completes the Bridge prior authorization; the decision comes within 72 hours of submission, mailed to you and sent to your prescriber (through the ePA portal or by fax). Part D plans and pharmacies don’t opt in, and you don’t enroll separately
- Use LillyDirect: Medicare patients can pay cash through LillyDirect ($149–$299/month) if you don't qualify for the Bridge
- Lilly Cares: Zepbound, Mounjaro and Foundayo are not on Lilly Cares’ medication list

For Qualifying Patients
Patient Assistance (Lilly Cares)
Lilly Cares is Eli Lilly's patient assistance program that provides free or reduced-cost medications to qualifying patients. However, Zepbound, Mounjaro and Foundayo are not on Lilly Cares’ medication list, so it is not a route to free or low-cost Foundayo.[4]
Foundayo Is Not on the Lilly Cares List
For Foundayo, your realistic options are the savings card (commercial insurance), LillyDirect self-pay ($149–$299/month), the Medicare GLP-1 Bridge (if you have Medicare Part D) or an insurance appeal. Lilly Cares (1-800-545-6962, lillycares.com) can help with other Lilly medicines on its list.
Income Eligibility Guidelines
For the Lilly medicines it does cover, Lilly Cares eligibility is based on household income as a percentage of the Federal Poverty Level (FPL):
| Household Size | Group 1 (300% FPL) | Group 2 (400% FPL) | Group 3-4 (500% FPL) |
|---|---|---|---|
| 1 person | $46,950 | $62,600 | $78,250 |
| 2 people | $63,550 | $84,800 | $106,000 |
| 3 people | $80,150 | $107,000 | $133,750 |
| 4 people | $96,450 | $128,600 | $160,750 |
Income limits based on Federal Poverty Level (FPL) and are updated annually. Visit lillycares.com for current thresholds.

Price Comparison
Foundayo vs Other GLP-1 Costs
How does Foundayo's pricing compare to other GLP-1 medications? For self-pay patients, oral options are significantly more affordable than injectables.
| Medication | Self-Pay Monthly | With Savings Card | Administration | Weight Loss |
|---|---|---|---|---|
| Foundayo | $149–$299 | $25 | Daily pill | ~12% |
| Oral Wegovy | $149–$299 | ~$25 | Daily pill (30-min fast) | ~15% |
| Injectable Wegovy | $349 | ~$25 | Weekly injection | ~15% |
| Injectable Zepbound | $299–$449* | ~$25 | Weekly injection | ~20-22% |
*Zepbound: $299–$449/month via LillyDirect self-pay (KwikPen or vials); ~$1,000/month at retail pharmacies without savings programs.
Oral = Lower Self-Pay Costs
The bottom line: with insurance and a savings card, all these medications cost about $25/month. For self-pay patients, the pills (Foundayo, oral Wegovy) generally cost less than the injectables, especially at lower doses.
Trade-Off: Convenience vs. Efficacy
Foundayo offers ~12% average weight loss compared to ~15% for Wegovy and ~20-22% for Zepbound. However, Foundayo has advantages that matter to many patients:
- No injections: Daily pill taken any time of day
- No food restrictions: Unlike oral Wegovy (requires 30-minute fasting)
- No refrigeration: Store at room temperature
- Lower self-pay cost: $149–$299/month vs $349/month (Wegovy injection) or $299–$449/month (Zepbound) self-pay

$149–$299/month through LillyDirect depending on your dose: $149 / $149 / $199 / $299 / $299 / $299 by dose (0.8 / 2.5 / 5.5 / 9 / 14.5 / 17.2 mg). The 2.5mg and 5.5mg prices are temporarily reduced through December 31, 2026. The 14.5mg and 17.2mg doses stay at $299 only if you refill within 45 days; otherwise the price is $349/month.
Coverage policies are still forming since Foundayo was just FDA-approved in April 2026. Some commercial insurers may cover it with prior authorization. If your insurance covers Foundayo and you use the free savings card, you can pay as little as $25/month. Check with your specific plan or have your doctor submit for prior authorization to find out.
To pay $25/month, you need: (1) commercial insurance that covers Foundayo, and (2) the free Foundayo Savings Card. Activate your card at foundayo.lilly.com or call 1-866-923-1953. Note: Government insurance (Medicare, Medicaid, TRICARE, VA) is not eligible for the savings card.
Yes, for self-pay patients. Foundayo costs $149–$299/month through LillyDirect. Zepbound is $299–$449/month via LillyDirect, or $1,000+ at retail pharmacies without a savings program. With insurance and savings cards, both can cost $25/month. The trade-off: Foundayo delivers ~12% weight loss (vs ~20-22% for Zepbound), but Foundayo is a convenient daily pill with no injections or food restrictions.
It already does, with conditions. Since July 1, 2026, the Medicare GLP-1 Bridge covers Foundayo for weight loss at $50 per monthly fill for eligible Part D enrollees, through December 31, 2027. Part D plans and pharmacies don’t opt in, and you don’t enroll separately; your prescriber must get prior authorization. Not eligible if you have type 2 diabetes, moderate-to-severe obstructive sleep apnea, or noncirrhotic MASH with moderate-to-advanced liver fibrosis (F2–F3), or if your Part D plan paid for a GLP-1 for you at any time in 2026. Medicare patients who don't qualify can pay cash through LillyDirect ($149–$299/month).
Foundayo costs: as little as $25 with commercial insurance + savings card, $149–$299/month through LillyDirect self-pay (dose-dependent), or $50 per monthly fill through the Medicare GLP-1 Bridge (eligible Part D enrollees, with conditions). The savings card option provides the lowest cost for commercially insured patients.
No. Foundayo (orforglipron) is a new branded medication that was just FDA-approved in April 2026. There is no generic available, and one is not expected for many years due to patent protection. Current options for savings include the savings card ($25/month with commercial insurance) or LillyDirect self-pay ($149–$299/month).
