IMPORTANT CLARIFICATION
Does Lilly Cares Cover Foundayo?
Foundayo Is Not Currently Covered by Lilly Cares
Lilly Cares, Eli Lilly's patient assistance program (PAP) that provides free medications to low-income patients, does not currently include Foundayo on its medication list.[1] This may change in the future, but as of April 2026, Foundayo is not available through Lilly Cares.
If you've seen information online suggesting Foundayo is covered by Lilly Cares at 300% FPL, that information is inaccurate. We've verified this directly with current program documentation.
What Is Lilly Cares?
Lilly Cares is Eli Lilly's charitable foundation that provides free medications to qualifying patients who:
- Are uninsured or underinsured
- Meet income requirements (typically 300-500% FPL depending on medication)
- Are U.S. residents with a valid prescription
While Lilly Cares covers many Lilly medications, Zepbound, Mounjaro and Foundayo are not on Lilly Cares’ medication list.[1]
Will Lilly Cares Add Foundayo in the Future?
Lilly has not announced any plan to add Foundayo, so don't count on Lilly Cares for Foundayo. If that changes, Foundayo will appear on the Lilly Cares medication list:
- The official Lilly Cares website lists the medicines it currently covers
- Lilly Cares' phone line is 1-800-545-6962 (for medicines on its list)
In the meantime, there ARE real options to make Foundayo more affordable—see below.
NOT A PAP, BUT A GREAT ALTERNATIVE
LillyDirect Self-Pay Option
Best Option for Uninsured Patients
LillyDirect isn't a patient assistance program—it's Eli Lilly's direct-to-consumer pharmacy offering Foundayo at reduced self-pay prices:
- 0.8 mg (starting): $149/month
- 2.5 mg (titration): $149/month (temporarily reduced through December 31, 2026)
- 5.5 mg: $199/month (temporarily reduced through December 31, 2026)
- 9 mg (maintenance): $299/month
- 14.5 mg, 17.2 mg (highest): $299/month*
*$349/month if not refilled within 45 days (45-Day Refill Rule)
No income requirements. No insurance needed. Shipping available now.[2]
These prices represent significant savings compared to retail—and unlike traditional patient assistance, anyone with a valid prescription can use it.[3]

ADDITIONAL WAYS TO REDUCE COSTS
Other Affordability Options
Appeal Your Insurance Denial
If your insurance denied Foundayo coverage, don't give up. Per KFF analysis cited by the AMA, over 80% of appealed prior authorization denials are overturned—yet only about 1 in 10 patients appeal.[4] An appeal can get your insurance to cover Foundayo, potentially reducing your cost to just a copay.

Independent Patient Assistance Foundations
Some nonprofit foundations provide financial assistance for medications independent of manufacturers:
- PAN Foundation — $1,000 grants for obesity treatment including GLP-1s: panfoundation.org
- NeedyMeds — Database of assistance programs: needymeds.org
- Patient Advocate Foundation — Copay relief programs: patientadvocate.org
- RxAssist — Comprehensive database: rxassist.org
Note: Availability for GLP-1 weight loss medications varies. Check each program for current coverage. Use FundFinder for alerts when funds open.
Check State Medicaid Expansion
Some state Medicaid programs are beginning to cover GLP-1 medications for weight loss. Coverage varies significantly by state and is subject to change.
See our Medicaid coverage guide →
Consider Alternative GLP-1 Medications
If Foundayo remains unaffordable, other GLP-1 medications may have different coverage or assistance programs:
- Wegovy — Novo Nordisk offers patient assistance programs
- Saxenda — May have different insurance coverage

Confirmed for 2026
TrumpRx Direct-Purchase Program
TrumpRx launched February 5, 2026 and routes patients to manufacturer direct-to-consumer sites. Check trumprx.gov for current availability.[9]
Medicare GLP-1 Bridge Program
The Medicare GLP-1 Bridge has run since July 1, 2026 (through December 31, 2027), with a flat $50 per monthly fill for Wegovy (injection and pill), Foundayo, and the Zepbound KwikPen, including Foundayo. Eligibility: Age 18+ and BMI ≥35; or BMI ≥30 with HFpEF, uncontrolled high blood pressure or CKD stage 3a+; or BMI ≥27 with prediabetes, a prior heart attack or stroke, or symptomatic PAD. FOUNDAYO AFFORDABILITY FAQS
Frequently Asked Questions
Not through Lilly Cares. While Eli Lilly's Lilly Cares patient assistance program provides free medications to qualifying low-income patients, Foundayo is not currently on the covered medication list. However, uninsured patients can use LillyDirect to get Foundayo at $149–$299/month, and commercially insured patients may qualify for the Foundayo Savings Card ($25/month).
Currently, there is no program offering free Foundayo. The lowest-cost options are: (1) Foundayo Savings Card at $25/month if you have commercial insurance that covers it, or (2) LillyDirect Self-Pay at $149/month for the starting dose if you're uninsured. Some independent nonprofit foundations may offer limited assistance—check NeedyMeds.org for current programs.
Eli Lilly has not publicly stated why Foundayo is excluded from Lilly Cares. It may be due to the high demand and cost of GLP-1 medications, or strategic business reasons. Lilly Cares covers some Lilly medications for diabetes, but Foundayo (orforglipron) for weight loss is excluded. This could change in the future—check the official Lilly Cares website for updates.
The cheapest legitimate option for uninsured patients is LillyDirect at $149/month for the 0.8mg and 2.5mg doses, $199/month for 5.5mg, or $299/month for 9mg and up (with on-time refills). The 2.5mg and 5.5mg prices are temporarily reduced through December 31, 2026. Shipping is available now.
Only if you have commercial insurance that covers Foundayo. The Foundayo Savings Card reduces your copay to $25/month at the lowest, but that price requires: (1) commercial/private insurance, (2) a plan that covers Foundayo, and (3) no Medicare, Medicaid, or other government insurance. If your commercial plan doesn't cover Foundayo, the card still applies: the price is the same as self-pay pricing ($149–$299/month), up to 10 fills. The 2026 savings card expires December 31, 2026.
Yes. Many patients use LillyDirect ($149–$299/month) to start treatment while their prior authorization or appeal is processing. If your insurance later approves coverage, you can switch to using insurance + the savings card ($25/month). This ensures you don't delay treatment while navigating the insurance process.
There are several ways to make Foundayo more affordable: (1) With commercial insurance: Use the Foundayo Savings Card to pay as low as $25/month. (2) Without insurance: Use LillyDirect at $149-$299/month (up to $349 for highest doses). (3) If denied by insurance: Appeal the denial—80% succeed per KFF analysis cited by AMA. (4) Check nonprofits: Organizations like PAN Foundation, NeedyMeds, and Patient Advocate Foundation may offer additional assistance.
