How to Get Foundayo Covered by Insurance

  1. Check eligibility: BMI 30+ (or 27+ with conditions like high BP, sleep apnea)
  2. Gather documentation: Weight history, diet/exercise efforts, medical records, and any prior medication attempts
  3. Doctor submits Prior Authorization: Most employer and ACA plans must decide within 15 days (72 hours if urgent)
  4. If denied, appeal: You have 180 days — 80%+ succeed
(Options: internal appeal → peer-to-peer review → external review)
  5. Need help with your appeal?

What This Guide Covers

There are paths forward:

  • Appeals work — 80%+ of patients who appeal their denial win coverage[1]
(and save thousands with access to $25/fill savings card)
  • Keep fighting — exceptions, peer-to-peer review, and external review if needed
  • Medicare Bridge is live — Foundayo for $50 per monthly fill through the Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027), if you meet CMS’s criteria[12]
  • Consider alternatives — If Foundayo coverage is restrictive, Wegovy may have better formulary placement at your insurer[11]
  • Self-pay options — LillyDirect from $149/mo[10]

Does Your Insurance Cover Foundayo?

HOW TO FIND OUT

Call Your Insurance Provider 📞

Call the number on your insurance card and ask:

  • "Is orforglipron (Foundayo) on my plan's formulary?"
  • "Do you cover it for weight management, or only for diabetes?"
  • "What tier is it on?" (lower tier = lower copay)
  • "Is prior authorization required?"

Pro tip: Write down the representative's name and any reference number. Verbal confirmations aren't binding, but documentation helps if there's a dispute later.

What Is a Formulary?

A formulary is your insurance plan's list of approved medications. If Foundayo isn't on your formulary, your plan won't cover it—even with a prescription.

The reality: Only 43% of firms with 5,000+ workers, and 16% of firms with 200–999 workers (2025), covered GLP-1 medications like Foundayo for weight loss (KFF).4 Many plans explicitly exclude weight-loss drugs entirely.

Common requirements: BMI ≥30 or ≥27 + comorbidity, 6-12 months lifestyle modification

⚠️ 2026 Open Enrollment Alert

Some employers and insurers (BCBS Massachusetts, Health New England, Kaiser) are dropping or restricting GLP-1 weight loss coverage for 2026. If you're choosing a plan now, verify Foundayo coverage BEFORE enrolling.

Ask your HR or insurer: "Does this plan cover orforglipron (Foundayo) for weight management in 2026?"

Which Insurers Cover Foundayo?

Coverage varies significantly by insurer and plan type. About 43% of large employer plans cover GLP-1 medications. For a complete breakdown of which insurers cover Foundayo and their requirements, see our insurance directory.

Medicaid Coverage Guide

State-by-State Coverage

Only 13 states (as of January 2026) covered GLP-1s for weight loss through Medicaid,8 and the landscape is shifting—some states are adding coverage while others are cutting it due to costs.9

BALANCE model (Medicaid): Voluntary for states, which can join between May 2026 and January 1, 2027; no participating state has been confirmed.12

Medicaid requirements are typically stricter than commercial plans (BMI ≥35-40 vs ≥30), and coverage can change mid-year. Always verify current coverage with your state.

Medicare Coverage Guide

Medicare Coverage Is Changing

Medicare and weight-loss GLP-1s: the Medicare GLP-1 Bridge

Through the Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027), a temporary CMS demonstration that runs outside the Part D benefit, people with eligible Part D coverage can get Wegovy, Foundayo or the Zepbound KwikPen for weight loss for $50 a month, if they meet CMS’s eligibility criteria and their prescriber gets prior authorization.

  • Who qualifies: Age 18+ and BMI ≥35; or BMI ≥30 with HFpEF, uncontrolled high blood pressure or CKD stage 3a+; or BMI ≥27 with prediabetes, a prior heart attack or stroke, or symptomatic PAD.
  • Who doesn’t: Not eligible if you have type 2 diabetes, moderate-to-severe obstructive sleep apnea, or noncirrhotic MASH with moderate-to-advanced liver fibrosis (F2–F3), or if your Part D plan paid for a GLP-1 for you at any time in 2026.
  • Cost: $50 per monthly fill. Does not count toward the Part D deductible or out-of-pocket cap. Extra Help (LIS) does not lower it. Not eligible for the Medicare Prescription Payment Plan. Coupons and discount programs can’t be applied.
  • No opt-in needed: Part D plans and pharmacies don’t opt in, and you don’t enroll separately; your prescriber must get prior authorization.
  • If it’s denied: No appeals process for a Bridge prior-authorization denial: your prescriber can resubmit with more information (Part D appeal rights for Part D coverage decisions still apply).

Does your employer plan exclude all weight loss meds?

If your employer-sponsored plan excludes all weight loss medications, you can advocate for policy change. Request that your HR department add GLP-1 coverage for the next enrollment period—and encourage coworkers to do the same.

Filing for Prior Authorization

HOW TO GET PREPARED

What Is Prior Authorization?

Prior authorization (PA) is your insurer's way of pre-approving expensive medications before they'll cover them. For Foundayo, this means your doctor submits documentation proving you meet the medical criteria—and the insurer decides whether to pay.

Why it matters: A well-prepared PA submission is the difference between approval and denial. Missing documentation, incorrect codes, or incomplete medical history are the most common reasons Foundayo PAs get rejected. Get the PA checklist →

What Your Doctor Needs

  • Current BMI (measured within 30 days)
  • Comorbidity diagnoses (hypertension, diabetes, sleep apnea, high cholesterol)
  • 3-6 months of lifestyle modification documentation
  • Prior weight-loss medications tried (if any)
  • Letter of medical necessity

Timeline: Most employer and ACA plans must decide within 15 days (72 hours if urgent). Follow up by day 5.

What Happens Next?

Most common: Many PA decisions arrive within days; the federal limit for most employer and ACA plans is 15 days. If approved, you'll get a confirmation letter or portal notification.

How to activate your Foundayo savings card

With insurance coverage: As low as $25/month

  • Max savings: $100/month (or $200 for 2-month, $300 for 3-month supply)
  • Annual limit: $1,000/year (max 10 fills)

Eligibility requirements:

  • Commercial insurance that covers Foundayo
  • NOT Medicare, Medicaid, VA, or TRICARE
  • U.S. resident, age 18+

Terms valid through December 31, 2026.

Activate here: foundayo.lilly.com/savings

See complete savings card guide →

Save Your Card Details

After activating, save your BIN, PCN, and Group number (pharmacy processing codes). You'll need these at the pharmacy.

What to say at the pharmacy

When picking up your prescription:

"I have a Foundayo prescription with prior authorization. My PA reference number is [NUMBER]. I also have a Lilly savings card—here's the BIN and PCN. Can you confirm my copay before filling?"

Common pharmacy issues and fixes:

"PA not on file"

Ask them to call insurance or your doctor to verify. Say: "The PA was approved on [DATE]. Can you call the insurance PA department to verify the authorization?"

"Savings card not working"

Confirm the BIN and PCN are entered correctly. If still not working, call Lilly support at the number on your savings card.

"Not in stock"

Ask when it will be available or try another pharmacy. Some patients have better luck with specialty pharmacies.

How to prepare for Foundayo renewal

Monthly tracking (for your records and renewal):

  • Weight (same scale, same conditions)
  • Blood pressure
  • A1C (quarterly)
  • Side effects or issues
  • Lifestyle activities (exercise, diet tracking)

60 days before expiration:

  • Contact your doctor's office to initiate renewal
  • Confirm you've met your plan's weight loss threshold (typically 5%)
  • Gather documentation showing your progress

Don't Wait Until the Last Minute

A reauthorization is a new review, so allow time (most employer and ACA plans can take up to 15 days). Starting early prevents gaps in your medication supply.

What Happens Next?

Most common: Your first fill goes smoothly once the savings card is set up. Keep your PA reference number handy for refills.

Strategies for Restrictive Coverage

FORMULARY & TIER EXCEPTIONS

If Foundayo is Not on Formulary → Formulary Exception

Your goal: Add Foundayo to your plan's covered drug list. What to do if Foundayo is not covered by insurance →

  • Your prescriber must initiate the formulary exception
  • Submit medical necessity documentation
  • Include your obesity-related complications and clinical evidence

Timeline: Individual and small-group ACA plans must decide within 72 hours (24 hours if urgent); employer plans follow their own exception process; federal claims rules allow up to 15 days

If Foundayo is High-Tier → Request a Tier Exception

Your goal: Move Foundayo from Tier 3-4 to Tier 1-2 for lower costs.

  • Contact your insurance's formulary exception department
  • Have your doctor submit a tier exception request
  • Include documentation of failed trials with preferred medications

Timeline: Depends on your plan; ask when you file

Appeals & Reauthorization

IF DENIED OR RENEWING

Why Appeals Matter

Initial denials are a filter, not a final answer. Over 80% of prior authorization appeals succeed1—yet fewer than 1% of denied claims are ever appealed.2 Insurers rely on patient inaction.

Just got your denial letter? Start with our Foundayo Denied crisis guide for what to do right now.

Common denial reasons include "not medically necessary," step therapy requirements, and Foundayo not covered by insurance. Each has specific appeal strategies that work.

Common Denials → Your Move

"Not medically necessary"Strengthen clinical documentation
"Step therapy required"Document why alternatives won't work
"Formulary exclusion"Request formulary exception
"Missing documentation"Resubmit with complete records

Timeline: Internal appeal 30 days → External review 45 days

What Happens Next?

Most common: Internal appeals take up to 30 days. External reviews take up to 45 days.

How to apply for Foundayo patient assistance

Lower-cost options instead: LillyDirect self-pay ($149–$299/month), or the savings card if you have commercial insurance.

See complete patient assistance guide →

TrumpRx direct-purchase option

TrumpRx launched February 5, 2026. TrumpRx lists the manufacturers’ own self-pay prices (not a separate, cheaper price); Foundayo orders go through LillyDirect:

  • Price: $149–$299/month (same as LillyDirect)
  • Who can use it: Anyone—not limited to Medicare/Medicaid
  • How it works: Visit trumprx.gov → redirects to LillyDirect

Same Pricing as LillyDirect

TrumpRx directs you to LillyDirect.com for Foundayo. The pricing is identical—$149–$299/month depending on dose. Use whichever site is more convenient.

What Happens Next?

Most common: LillyDirect self-pay is the quickest way to start without insurance (home delivery or Walmart pickup). Zepbound, Mounjaro and Foundayo are not on Lilly Cares’ medication list.

Alternatives

OTHER GLP-1 OPTIONS

If Foundayo remains inaccessible, several FDA-approved alternatives like Wegovy and Saxenda, plus off-label options, may provide similar benefits depending on your insurance coverage.

Foundayo vs Wegovy and Saxenda

While clinical trials have demonstrated Foundayo to be the most effective GLP-1 for weight-loss yet, it's not your only option. Wegovy (semaglutide) and Saxenda (liraglutide) are two other GLP-1s approved by the FDA for weight loss.

When deciding which GLP-1 is right for you, consider your weight loss goals, insurance coverage, side effects and budget.

Foundayo vs Wegovy →

Foundayo vs Mounjaro and Ozempic

While Mounjaro and Ozempic are currently only approved by the FDA for treatment of Type 2 diabetes, it doesn't necessarily mean you won't be able to access these medications.

Off-label prescription of these medications for reducing the risks of developing type 2 diabetes or for treatment of metabolic syndrome have been successful for some—but this depends highly on your insurance plan and clinical profile.

COMMON COVERAGE QUESTIONS

Frequently Asked Questions

Start by calling your insurer to check if Foundayo is on your formulary. If covered, your doctor submits a prior authorization (PA) with your BMI, comorbidities, and documentation of lifestyle changes. Follow up within 3-5 days. See our prior authorization guide for detailed requirements by insurer.

Gather before your appointment: current weight/height (for BMI calculation), list of comorbidities with diagnosis dates, documentation of lifestyle changes (diet logs, gym records), and history of prior weight-loss medications tried. See our PA checklist for a complete list.

Most employer and ACA plans must decide within 15 days (one 15-day extension allowed; 72 hours if urgent), and many decide sooner. Call to check status by day 5—don't wait for a letter. If no decision after 7 days, request escalation. See our PA guide for timelines by insurer.

You have the right to appeal—and appeals work. Get your denial letter, note the exact reason, and see our appeals guide for step-by-step instructions.

Foundayo is already covered by the Medicare GLP-1 Bridge (July 1, 2026 – December 31, 2027) for $50 per monthly fill, if you have Part D coverage and meet CMS’s eligibility criteria. If you don’t qualify, LillyDirect offers self-pay at $149–$299/month. See our Medicare guide for details →

Yes. Many patients use LillyDirect ($149–$299/month) to start treatment while their PA or appeal processes. If insurance approves later, you can switch to using your coverage. See our cost guide for all payment options.

Start the renewal process 60 days before your authorization expires. Most insurers require proof of 5% baseline weight loss to continue coverage. Track your weight monthly. Your doctor will submit updated documentation showing your progress and continued medical necessity.
